议甘麦大枣汤中小麦的选择

甘麦大枣汤源自张仲景《金匮要略 ·妇人杂病脉证并 治》，原文云： “妇人脏躁，喜悲伤欲哭，象如神灵所作，数欠 伸，甘麦大枣汤 主 之。甘 草 三 两，小 麦 一 升，大 枣 十 枚，上 三 味，以水六升，煮取三升，温分三服。亦补脾气。”条文提出了 用甘麦大枣汤治疗“脏躁”。后世医家多用此方剂，作为治疗 精神情感障碍类疾病的心、肝、脾虚证的基础方剂，随证加减 运用。笔者搜索 2001 年 1 月至 2011 年 12 月 153 篇临床文 献报道中发现该方组成中的“小麦”一味，有 53 篇中显示选 用小麦，有 46 篇显示选用浮小麦，有 44 篇文献中对“小麦” 一味并未明确。

摘要： 目的： 对张仲景治疗脏躁证的经典方剂甘麦大枣汤中“小麦”一味的选择进行探讨。方法： 通过对有关文献报道的分析，
从小麦与浮小麦饮片的考证、甘麦大枣汤的病机主治和组方原理三方面进行探讨。结果： 小麦和浮小麦是两种具有不同性
味、归经和功效的中药饮片； 小麦以养心补脾为长，更符合甘麦大枣汤所治脏躁之证的病机。结论： 甘麦大枣汤中选择小麦比
笔者认为临床选用“小麦”一味是否选用不可混淆，现就 甘麦大枣汤中“小麦”一味的选择从饮片考证、主治症及组方 原理三方面进行分析探讨。

浮小麦更为适合，更适用于脏躁一证，也更为符合张仲景遣方用药原意，临床宜区别使用。
Abstract： Objective： To explore the the slection of“Wheat”in Ganmai Dazao Decoction，a classic formula used by Zhang Zhongjing in treating Zangzao syndrome． Methods： Through the analysis of the related literature，the discussion was conducted from three areas of the textual research of wheat and light wheat Yinpian（ herbal pieces prepared for decoction） ，the pathogenesis of indications and prescriptions principle of Ganmai Dazao Decoction． Results： Wheat and light wheat weree two different Yinpian of Chinese herbal medicine with different tastes，meridian tropism and efficacy； wheat was sapeiclized in nourishing the heart and replenishing the spleen，which was more appropriate for the pathogenesis of Zangzao syndrome indicated by Ganmai Dazao Decoction． Conclusion： The selection of wheat in Ganmai Dazao Decoction is more suitable than the light wheat，which is more proper for Zangzao syndrome，and more in line with the original intent of the Zhang Zhongjing's medication and prescription，so the use between them should be differentiated in clinic．

1 从小麦与浮小麦饮片的考证议
小麦与浮小麦两者均为我国传统习用中药饮片。小麦 为禾本科植物小麦（ Triticum aestivum L． ） 生长成熟的干燥果 实，处方中多写为淮小麦、陈小麦； 而浮小麦则为小麦未成熟 干燥轻浮瘪瘦的果实，处方写为浮小麦［1］。
