起重司索指挥的定义与职责
定义:起重司索指挥是指在起重作业中 ,负责指挥起重机操作、指导吊装作业 的人员。

Z HONGYUAN P ETROLEUM E XPLORATION B URE AU ZPEB UGANDA CO. LIMITED关于对TOT AL 操作模式的几点认识一、 设备吊点检测要求所有需用吊车吊装的设备,哪怕是一个钻台梯子或滑板等小件设备,在进入甲方作业区之前都必须出示吊点检测合格证(分别为吊点双重负荷测试合格证及MPI 磁粉探伤合格证),我们的所有设备及营房仅有出厂合格证,而没有针对吊点的专门检测报告或合格证书,即使部分设备有探伤报告也不被道达尔所认可,道达尔只接受带有CE标志的合格证书。
如果不能出示甲方认可的吊点检测合格证,那么就必须让道达尔认可的有资质的第三方检测公司来检验(Pre-Mpi 、Load Test 、Post Mpi)。
目前在乌干达甲方认可的是一家英国检测公司Lloyds British 。
按照该公司的程序文件规定,对于吊点负荷测试及探伤合格的设备,每半年进行一次目测复检、每年一次MPI 复检、每两年一次Double Load Test 。
对于双重负荷测试,可以是两个相同重量的物体叠加在一起测试、也可以对4个吊点的设备采用对角吊2个吊点来测试。
附:Lloyds British 检测公司的检测合格铭牌、探伤合格证书、负荷测试证书。
乌干达RIG26队从12月13日卸第一车设备开始,直至12月31日结束,历时19天才完成了所有大小设备的吊点检测工作。
检测地点为甲方作业区外的一个当地公司营地内,道达尔负责吊装作业的主管CPLO(Competent Person For Lifting Operation)派属下全程监督检测过程。
甲方对吊点的要求极高,从焊接的质量、外表是否美观、吊点分布是否合乎规范、吊耳的厚度孔径等规格是否与被吊物体的吨位匹配、4个吊耳相互对应的角度是否合理等,都要进行系统的检测。
值得一提的是国内设备喜欢做的那种开放式吊耳(振动筛、除气器等设备就是这类吊耳)绝对是不允许使用的,所有这一类型的吊耳都被要求割掉,并重新加工了厚度与孔径以及圆孔上部高度相匹配的吊耳之后,将吊耳焊在小块钢板上,然后将钢板再焊到被吊物体上才给予检测。

1. 编制目的Purpose为规范吊装作业,防范因麻痹大意、误操作、违章操作等引起安全事故,结合我公司实际制定本制度。
2. 适用范围Area of Application本制度适用于在XX中国设施工作的所有员工、供货商和承包商。

吊装物体虽不足5t,但形状复杂、刚度小、长径比大、精密贵重,以及在作业条件特殊的情况下,也应编制吊装作业方案、施工安全措施和应急救援预案。
5.2吊装方案应至少包括:吊装物及吊装设备概况、吊装屏幕布置图、工艺计算(包括受力分析与计算、机具选择、起吊物构件校核等)、作业人员组织、安全技术措施、风险评估与应急预案。
如设计设备装备的,还需要补充施工步骤及工艺布置。
6. 过程描述Process Description6.1 作业前的安全检查吊装作业前应进行以下项目的安全检查:6.1.1 实施吊装作业的所属单位或外来施工单位要组织相关人员对起重吊装机械和吊具进行安全检查和确认,确保设施设备状态完好。
6.1.2 实施吊装作业单位要组织人员对吊装区域内的安全状况进行检查(包括吊装区域的划定、标识、障碍)。

WHEN THE CRANE OPERATOR IS UNSIGHTED.
TAKE THE WEIGHT OF THE LOAD SLOWLY,INCH THE LOAD UNTIL THE FULL WEIGHT IS ON THE CRANE AND THEN GRADUALLY INCREASE THE SPEED
2. RELEASE TENSION ON SLINGS BUT LEAVE CRANE ATTATCHED. GIVE LOAD A GOOD PUSH ALL ROUND TO DOUBLE CHECK LOAD WILL NOT MOVE
3. WHEN COMPLETELY SATISFIED THAT THE LOAD WILL NOT MOVE / TIP / SLIDE.UNHOOK CRANE
THE SLINGER DETERMINES THE WEIGHT OF THE LOAD AND DECIDES THE SAFE AND CORRECT METHOD OF SLINGING.HE MUST ALSO BE FAMILIAR WITH THE CRANES LIFTING CAPACITIES.
STOP !
HAVE BANKSMEN AS BACK UP.
ALWAYS TEST BOTH THE RADIOS WITH ONE ANOTHER. KEEP MESSAGES CLEAR AND RELEVENT. REMOVE GLOVES
KEEP RADIO OUT OF WIND. AVOID USING IN HIGH NOISE AREAS. CHANGE BATTERY PACK AT REGULAR INTEVALS IF IN PROLONGED USE
