本表仅作为申请在中华人民共和国境内做博士后的书面申请材料。

申请人必须已经 获得博士学位（或已通过博士论文答辩）。

填写本表必须实事求是，认真详实全面， 可打印或用黑色钢笔填写。

申请人必需向申请做博士后的单位提交两份《专家推荐信》。

申请人在下栏中填好自己姓名和所申请的单位名称后，将此表分送两位了解和熟悉自己的专家（其中一位最好是申请人的博士生指导教师）。

为方便专家填写推荐意见后将其退回申请人，请申请人在下面的栏目中详细写明自己的通信地址。

Applicant should have two letters or recommendation submitted from professors or others who can assess the quality of his/her academic performance, capability and potential of research. Please ask to have these letters sent directly to the institution to which you are applying or returned to you with the envelope sealed.

推荐意见※
凡在国外留学的博士，申请回国做博士后时，需将此《推荐意见》表送我驻外（申请者留学国家）使领馆教育处（组）填写推荐意见。

申请者填好第一栏的内容后送使领馆教育处（组）。

使领馆教育处（组）填写推荐意见后，直接（也可由申请人本人）将其和其它申请材料一起送申请人所在的单位或全国博士后管委会办公室。

留学博士回国做博士后意向书
凡在国外留学的博士，申请回国做博士后时，如果还没有与国内的单位（设站单位或非设站单位）取得联系，而需要全国博士后管委会办公室协助联系其回国后做博士后的单位，应将本意向书和其它申请材料（最好还有能反映申请人当前研究工作的论文资料）寄给全国博士管委会办公室。

单位审批意见
此表用于博士后流动站设站单位有关部门填写审批意见，也可作为非设站单位审查留学博士申请博士后的意见表。

当同意招收申请人做博士后时，需将此表和其它申请材料一起报全国博士后管委会办公室或博士后工作试点省市的人事、科技干部管理部门核查备案。
