Name Pre-Approval Application Form

Note: Please read the Filling Guidance with full attention, and fill the form accordingly.

Filling Guidance

Note: the following “guidance” can be only used for the purpose of filling illustration.

No need to be provided to registration administration.

This application form shall be applied to all the name pre-approval application, name modification application, name adjustment approval application (exclusive investors) and name extension approval application.

1.Only the related blanks of this application form should be filled.

2.The applicant should apply for name pre-approval and provide authentic, legal and valid

materials pursuant to Provisions on Administration of Enterprise Name Registration and Measures for the Implementation of Administration of Enterprise Name Registration.

3.“Type of business” should be filled under: limited liability company, limited liability

company by shares, subsidiary, non-company legal person, operation entity, solo proprietorship, partnership and others.

4.“Scope of business” should be filled with main business scope which matches the

company name and be subject to national standard Categories of National Business Scope.

5.The applicant for the application of name pre-approval, approved name adjustments or

extension should be all investors. All individual investors should sign their names for the application and all enterprises investors should seal the application. For the name pre-approval application, if the applicant is the former company, the application should be signed by the legal representative or execution partner or sealed by the company.

6.The item like registration address or registered capital could be modified without the

change of the approved name but the investors are excluded.

7.Extension of notification of Name pre-approval application and Name change

pre-approval application should be submitted one month before the expiration date.

When applying for the extension, the original copy of notification of Name pre-approval application and Name change pre-approval application should be submitted. The investors could apply for the extension for the duration of 6 months with noble cause and the extension could only be applied once.

8.The representative or the age nt should sign “match the original one” when they paste

their ID copies.

9.The item “investors” and “Approved name adjustments items (exclusive inv estors) could

add more lines or pages to fill all items.

10.The application should be in A4 form. It should be signed or filled with black pen or

ball-pen.
