产科手术,特别是困难的人工流产和上环取环等。但由于 经腹部超声的操作方式需要患者在手术过程中保持膀胱充 盈,患者相对比较痛苦,超声图象效果不理想,手术中一 旦采用麻醉容易造成尿失禁等现象,并且还需要另一操作 者用手持续掌控超声探头,极大的浪费了宝贵的医疗资源, 所以,经腹部探查方式在妇产科手术中的应用受到了很大 的限制。

妇科门诊仪器操作说明
可视人流仪器的操作方法1、打开主机开关。
2、检查负压瓶是否安装好。
负压控制踏板是否正常3、由巡回护士把一次性保护胶套窥器放在手术内。
将探头上涂上超声耦合剂。
4、手术医生带好手套,查清子宫位置。
5、消毒外阴。
6、用手指将一次性胶套套在探头上,并向下拉胶套,使其与探头紧密贴合。
7、将套好的探头嵌装在窥器上,合其与窥器紧密配合。
8、放叶窥器开后穹隆处,消毒阴道。
用宫颈钳夹位宫颈,将子宫昼拉到水平位,9、稍微活动窥器,使子宫显像清楚。
基物固定宫颈钳的同时固定窥器。
使图像稳定清晰,即可开始手术。
B超监视妇产科手术仪一、手术吸引器使用方法:1 、吸引器使用前,首先要调整好适合工作的负压值。
方法是:先堵住储液瓶吸气口,调整“负压调节”旋钮,通过负压表观察负压值。
“负压调节”钮顺时针旋转负压值升高,旋紧时将达到该机极限负压。
2、当吸引器调到某一负压时,如果负压泵停止运转,在止逆阀作用下仍可保持储液瓶内的负压值。
在吸引器操作过程中,可预选设置一个适合的负压值。
若在停机吸引过程中感到吸引力不足,可随时开动机器,提高负压值。
3 、吸引器的动转开关和脚踏开关可任意选用。
4 、使用时应注意不要使第二个储液瓶的液面超过吸液管。
如液体载入了防倒阀时,吸引力将消失,此时必需停机排倒液体,待冲净各部分后,重新装上方可使用。
5、如无特殊情况,请不要选用极限负压值。
6、在流产吸引中,如发生特殊情况应立即中上吸引时,可拉起“泄放钮”,负压会迅速下降至零位。
“泄放钮”,复位(放下按钮)后,恢复正常工作。
二、注意事项1、停机前,建议使吸引管吸入少量的清洁水以清洗管道的内壁。
2、停机后,倒贮液瓶,用柔软的刷子或抹布清除瓶和瓶塞上的污垢,再用清水冲洗。
3、贮液瓶瓶塞及各种管道可以用康威达消毒片按1:500浓度配制的消毒液浸泡1小时。
4、机箱外表面用浸过消毒液的微湿抹布来擦拭,防止液体渗入机箱缝隙。
5、金属材料的吸引管可用温度为134的饱和蒸气,保持20分钟进行消毒,灭菌。

BELSON700系列全程超导可视妇产科手术仪在计划生育手术中的应用目的:探讨BELSON700系列全程超导可视妇产科手术仪在计划生育手术中的优势。
方法:分别采用腹式B超、BELSON700系列全程超导可视妇产科手术仪及传统盲视手术方法进行比较分析。
回顾分析了早孕合并高危因素受术者121例,其中BELSON700系列全程超导可视妇产科手术仪引导下的人工流产术31例为观察组,腹式B超引导下的人工流产术30例为对照组1,常规人工流产术60例为对照组2。
比较观察组与对照组术中术后并发症情况。
结果:观察组平均手术时间(2.5±1.3) min,平均术中出血量(5.5±2.4) ml;对照组1平均手术时间(2.8±1.3) min,平均术中出血量(5.7±2.5) ml,对照组2平均手术时间(5.6±1.1) min,平均术中出血量(10.2±2.0) ml,观察组与对照组1之间差异无显著性(P>0.05),观察组与对照组2之间差异有显著性(P<0.05)。
结论:可视下计划生育手术准确可靠,但就可操作性来讲,BELSON700系列全程超导可视妇产科手术仪要优于腹式B超。
[Abstract] Objective:To analyze the advantage of BELSON700 in family planning.Methods:Early pregnancy 121 cases with high risk factors as samples.31 cases artificial abortion in BELSON700 were choosen as test group,30 cases and 60 cases were choosen as control pared BELSON700 with B-ultrasonic in artificial abortion and traditional artificial abortion.Results:The mean operation duration of test group and control group were(2.5±1.3) min,(2.8±1.3) min and (5.6±1.1) min respectively.The mean bleeding volumn were(5.5±2.4) ml, (5.7±2.5) ml and(10.2±2.0) ml respectively.There was no deference between the test group and the control group 1.There was significant deference between the test group and the control group 2.Conclusion:It is safe for visible family planning operation,but it is more convenient with BELSON700 than with B-ultrasonic in Artificial Abortion.[Key words] BELSON700;B-ultrasonic in artificial abortion; Early pregnancy; Artificial abortion人工流产术是目前广泛用于避孕失败后的补救措施,近年来手术质量不断提高,但手术的并发症时有发生,特别在早孕合并畸形子宫、瘢痕子宫、哺乳期子宫、子宫过度屈曲等高危因素者要求人工流产时,手术难度明显加大,又加上常规人工流产术在盲视下施行,全凭术者的经验与手感操作,存在一定的盲目性,易发生手术并发症,给受术者身心造成不同程度影响。

B型超声波无疑是满足上述需要的最具潜力的手段之一。首先,超声波对 于医生和患者都是安全的,不需要外加的防护;其次,超声波具有穿透 成像的特点,可以在体表检查,也可以在人体自然腔道或术中器官表面 进行扫查成像,而且不受出血影响;第三,不需要造影剂,超声可以显 示血管;第四,现代电子技术的发展,使得超声波仪器可以设计成符合 手术环境的特殊形态以满足术中的监视引导。 妇产科是超声波仪器使用率较高的临床科室之一。目前妇产科人工流产 类手术大都是在盲视状态下进行的,操作人员往往依靠通过器械传来的 “感觉”和经验来进行操作。但是,若操作人员对子宫的形态、大小判 断不准确的话,很可能对子宫造成不必要的损伤,尤其在出现子宫畸形、 宫颈狭窄折曲或子宫肿瘤(子宫内膜瘤等)的情况下对子宫的危险性更大。

经阴道探查、监视方式:
经阴道对妇产科手术进行超声监视已在临床得到了很好的评价,腔内 探头紧贴患者阴道前、后穹隆部位,避免了因腹壁脂肪太厚、肠道气 体干扰及膀胱充盈不足造成成像不佳的缺陷,使盆腔内声像探查更为 清晰;如一些肌层大肌瘤、黏膜层肌瘤,可因紧贴甚至压迫孕囊,在 手术中就容易损伤肌瘤,部分可导致肌瘤变性,同时经阴道超声还能 清晰显示子宫肌瘤位置、大小与孕囊的关系,所以可最大程度避免手 术对人体的损伤,经阴道超声对微弱的心管搏动、卵黄囊的显示率都 要高于腹部超声,而对孕囊过小、子宫畸形、子宫过度倾屈等造成临 床手术不完善的病因诊断更具优越性,将人工流产等手术的危险性及 不完善程度降到最低,对带环受孕患者行人流及取环手术尤有帮助。
