Instruction for the Application Form

1、若手写请使用钢笔或碳素笔，字体工整字迹清楚

Please complete all items with a black pen neatly and clearly, in the case of filling by hand.

2、表栏空格不能空白，无此项时应划“/”线

Please do not leave any items blank, fill in “/”with the items which are not applicable.

3、“11.申请方承诺”是认证中心根据国家产品认证有关法律、规范及标准等拟定,申请方应尽最基本的义务

Section 11 “Promises from applicant” is constituted by CCAP according to relevant laws, criterions and standards etc. of national product certification, applicant shall do the basic duties.

4、申请人应将申请书邮寄至中汽认证中心，地址：中国北京市西城区木樨地国宏大厦B座，邮编：100038

Applicant shall post the application form to CCAP, the address is “Guohong Building B, Muxidi, Beijing, 100038 China.

Note:

1、产品结构型式应注明安全带为简易式/卷收器式；两点式/三点式；两点式的是腰带/肩带；卷收器式的锁止型式。

For …Structure Type‟, please indicate safety belts are connivance type or retractor type; two-point belt or three-point; lap belt or shoulder belt in the case of two-point; retractor locking type.

2、织带的类型应注明是腰带/肩带/连续带。

For …Strap Type‟, please indicate the straps are lap s trap, shoulder strap or continuous strap.

Appendix 3

汽车安全带产品认证单元划分及申请免检时，申请企业应提供如下资料：

Applicant shall provide following materials, when the units division and auto-pass application of vehicle safety belts product certification are taking place:

1、申请企业应明确申请“3C”认证的产品有多少种型号，应划分为几个单元（按“产品认证实施规则”，包括新申请认证和进行转换的产品）。

Applicant should firstly confirm the number of types of products which are applying for “3C” certification and the number of units needed to be divided. (follow the Implementation Regulation of Compulsory Certification for Vehicles, include new product applications and transforming products)

2、一个单元中只有1种产品时，企业只需提供“申请认证汽车安全带产品规格型号汇总表”和“申请认证汽车安全带产品调查表”即可。

Enterprise only need to provide the “Collection Table of Model and/or Specification of Vehicle Safety Belts” and the “Products Survey for Compulsory Certification of Vehicle Safety Belts”, if there is only one product in a unit.

3、一个单元中有多种产品（2种或2种以上）时，除提供“申请认证汽车安全带产品规格型号汇总表”和“申请认证汽车安全带产品调查表”外，企业还应提供①主导产品的总成装配图（附彩色照片或图片）；②卷收器结构图；③文字形式说明其它产品与主导产品的相同点和不同点；④不同处的图形资料。

If there are multiple products (two or more) in a unit, apart from the two forms mentioned above, Enterprise needs to provide (1)Main products‟ assembly diagram (with color photos); (2)Graph of retractors‟ structure;

(3)Explanation in words of same and different points between main products and others; (4)Picture files of the different points.

4、企业申请免做部分检验项目（如不同单元的2种产品所用部件完全相同，可申请免做该部件检验）时，首先应明确提出具体要求，其次应提供免检项中所涉及部件的说明性资料（图形、材料、工艺、结构等）。

Enterprise should firstly put forward detailed requests; secondly provide illustrative documents (graphs, materials, processes and structures, etc.) of components involved in the auto-pass items, when applying for releasing some test items (you can apply for releasing the test of the components if they are used in 2 products from different units)
