清热凉血解毒法治疗寻常型银屑病血热证的重复测量分析_王俊慧

瘀血；气阴两虚，运血无力，气滞血瘀，又至阴虚燥热；阳虚不能化气行水、聚湿生痰，痰凝血瘀，阻遏经络，筋脉失养；另一方面，消渴日久，导致人体气血阴阳的平衡失调，风、寒、湿、热、毒邪乘虚而入，外邪阻络，气血不不通，亦致血脉瘀阻。脉络瘀滞，不通则痛；瘀血阻络，筋脉失养，不能濡养及温煦肢体、肌肉、筋骨，则皮肉枯槁，坏死脱落；瘀血日久不除，则郁而化热，蕴毒成脓，则见患肢溃烂流脓，终见组织坏死，趾（指）脱落。

总而言之，从证型的临床表现、病因病机及处方用

通讯作者：刘瓦利（1956－），女，北京人，教授、主任医师，博士研究生导师，研究方向：鳞屑性及脂溢性皮肤病的中西医研究。药来看，无论是气阴两虚型、湿热壅盛型、热毒炽盛型、阳虚寒凝型、阴虚热盛型、血瘀阻络型仰或是阴阳两虚型，多具有“气虚”、“瘀血”的特点。虽说其治疗上应标本兼顾，内外结合，根据病情发展的不同阶段辨证治疗，但是辨证治疗时不论何期何型，益气、活血、化瘀应贯穿其始终。

中医学辨证着眼于个体化，主要通过四诊收集病人的基本资料、症状、体征等信息，通过分析把握疾病全过程的特点及变化规律，然后概括为某种证型，进行相应治疗。辨证主要依靠所收集到的信息、个人的学识、及经验进行。带有个人主观色彩，虽说通过治疗的疗效可以验证辨证的正确与否，但是样本量过少，并缺乏代表性。本文通过大量广泛的收集文献资料，扩大样本量，使得所得结果更具有普遍的共性。但另一方面，仍然存在辨证的个体化、文献本身存在的问题（如不同文献间药物疗效性质、检验标准的不同等）、修正证型分析等问题，这对本文的结果具有一定影响。

银屑病血热证的重复测量分析

摘要：目的：客观评价清热凉血解毒法治疗寻常型银屑病血热证的临床疗效，并分析临床疗效趋势及特点。方法：采用重复测量的临床试验设计方法，观察寻常型银屑病血热证患者，采用清热凉血解毒法治疗后，PASI评分的变化特点，从中提炼治疗中医药治疗银屑病的优势所在及时间变化趋势。结果：71例入组患者中痊愈4例，显效15例，有效44例，无效8例；痊愈率6%，显效率21%，总有效率为89%。经重复测量分析，患者PASI积分的变化在治疗过程中呈平稳下降趋势。结论：清热凉血解毒法效果确切，而且通过重复测量的分析方法进一步明确了在治疗过程中的皮损变化特点及趋势，动态地描述了病情与治疗时间的相关性，为以后的相关研究提供了借鉴。

The Repeated Measures Analysis for Clinical Effectiveness of removing pathogenic heat from the blood in treating psoriasis vulgaris with Blood-Heat Syndrome

Abstract：Objective：To evaluate the clinical efficacy of removing pathogenic heat from the blood in treating psoriasis vulgaris with Blood-Heat Syndrome，and to analysis the trends and characteristics of the clinical efficacy．Methods：Applying repeated meas-ures design and analysis for this trail，to observe the PASI score varying characteristics of psoriasis vulgaris with Blood Heat Syn-drome after the treatment of removing pathogenic heat from the blood，and to refine the treatment of the advantages of Chinese medicine treatment of psoriasis location and time trends．Results：71cases finished the course of treatment；4patients（6%）were cured，15patients（21%）were effected excellently，44patients（62%）has effective results and8（11%）patients ineffective．The total effective rates of the three groups are89%．Through the repeated measures design and analysis，the changes of PASI in the course of treatment showed a steady downward trend．Conclusion：This study proved that there is a good curative effectiveness of treating psoriasis vulgaris by removing pathogenic heat from the blood，further more，applying the repeated measures analysis，we definitude the changing characteristics and trends of the lesions in treating，made a dynamic description of relevance between the disease severity and the treatment time，and gained experience for future allied research．
