①一般情况“普惠制产地证”填制说明普惠制产地证书标题栏〈右上角〉,填上检验检疫机构编定的证书号。
在证头横线上方填上“中华人民共与国”。
国名必须填打英文全称,不得简化。
Issued in THE PEOPLE'S REPUBLIC OF CHINA〈国内印制的证书,已将此印上,无须再填打。
〉第一栏：出口商名称、地址、国家。
例:CHINA ARTEX (HOLDING) COPR. GUANGDONG CO.NO.119, LIUHUA ROAD, GUANGZHOU, CHINA注:此栏带有强制性,应填明详细地址,包括街道名、门牌号码等。
中国地名的英文译音应采用汉语拼音。
如GUANGDONG (广东)、GUANGZHOU （广州）、SHANTOU （汕头）等等。
第二栏:收货人的名称、地址、国家。
该栏应填给惠国最终收货人名称（即信用证上规定的提单通知人或特别声明的受货人）,如最终收货人不明确,可填发票抬头人。
但不可填中间转口商的名称。
欧洲联盟、挪威对此栏是非强制性要求,如果商品直接运往上述给惠国,而且进口商要求将此栏留空时,则可以不填。
第三栏：运输方式及路线〈就所知而言〉例:ON/AFTER NOV.6, 2000 FROM GUANGZHOU TO HONG K0NG BY TRUCK, HENCE TRANSHIPPED T0 HAMBURG BY SEA.注:一般应填装货、到货地点〈始运港、目的港〉及运输方式〈如海运、陆运、空运〉。
转运商品应加上转运港,如VIA HONGKONG。
该栏还要填明预定自中国出口的日期,日期必须真实,不得捏造。
对输往内陆给惠国的商品,如瑞士、奥地利,由于这些国家没有海岸,因此如系海运,都须经第三国,再转运至该国,填证时应注明。

CONDITIONS: To be eligible for the preferential tariff treatment under the Regional Comprehensive Economic Partnership Agreement (the Agreement), goods should:a. fall within a description of goods eligible for concessions in the importing Party; andb. comply with all relevant provisions of Chapter 3 (Rules of Origin) and if applicable, Article 2.6 (Tariff Differentials) of Chapter 2 of the Agreement.2. EXPORTER AND CONSIGNEE/IMPORTER: Provide details of the exporter of the goods (including name, address and country) and consignee/importer (including name,address, and country) in Box 1 and Box 2, respectively.3. PRODUCER: Provide the details of the producer of the goods (including name, address and country) in B ox 3, if known. In case of multiple producers, indicate “SEE BOX 8” inBox 3 and provide the details in Box 8 for each item. If the producer wishes the information to be confidential, it is acceptable to state "CONFIDENTIAL", however, the producer information may be available to the competent authority or authorised body upon request. 普惠制原产地证明书普惠制原产地证明书普惠制原产地证明书(GENERALIZED SYSTEM OF PREFERENCE)，简称普惠制(GSP)，是指发达国家给予发展中国家或地区在经济、贸易方面的一种非互利的特别优惠待遇。
即发展中国家向发达国家出口制成品或半制成品时，发达国家对发展中国家予以免征或减征关税。
实施普惠制必须遵循三个原则，即：1、非歧视性原则(NON-DISCRIMINATION)即把原先称作落后国家、不发达国家或新兴国家统称为“发展中国家”(DEVELOPING COUNTRIES)，亦称“受惠国”(BENEFICIARY COUNTRIES);而把过去称作的先进国家或工业化国家一律成为“发达国家”(ADVANCED COUNTRIES)或“给惠国”(PREFERENTIAL GIVING COUNTRIES)。
2、惠遍原则(GENERALIZED)即对发达国家由发展中国家所进口的初级产品、半成品及商品实行普遍优惠制，给予普遍的、无例外的、不厚此薄彼的、一视同仁的优惠待遇。
3、非互惠原则(NON-RECIPROCITY)即是发达国家给予发展中国家或地区的普遍优惠，而不要求发展中国家给予发达国家提供反响优惠。

通过电子办证系统申办的证书的证书号由16个字符组成。

原产地规则（Rules of Origin） 是普惠制的核心组成部分，规定了受惠国出口到给 惠国的产品享受普惠待遇的必备条件。 原产地规则是衡量受惠国出口产品是否取得了原产 地资格，能否享受优惠关税待遇的标准。 各国给惠方案中的原产地规则虽然各有不同特色， 但都包括三个基本内容：原产地标准、直运规则、 书面证明。
FA CO FE是什么意思
FA/CO/FE是什么意思FA: 即Form A,是普惠制原产地证明书格式A，它的全称是《普遍优惠制原产证明（申报与证明之联合格式）格式A》，英文为：GENERALIZED SYSTEM OF PREFERENCES CERTIFICATE OF ORIGIN（COMBINED DECLARATION AND CERTIFICATE）FORM A，它是受惠国的原产品出口到给惠国时享受普惠制减免关税待遇时必备的官方凭证，所有的给惠国都接受。

FE:即Form E，是中国-东盟自由贸易区优惠原产地证明书，有"FORM E"出口6国减税只要签发了中国-东盟自由贸易区优惠原产地证实书（简称FORME），出口到文莱、柬埔寨、印度尼西亚、老挝、马来西亚、缅甸、菲律宾、新加坡、泰国、越南10国的货物可享受优惠进口国关税待遇，平均减税幅度达5%。
记者近日了解到，相当一部分企业对中国-东盟自由贸易区的这个“游戏新规则”不太了解，从中获益的企业不多。
据了解，2005年7月，中国与东盟的货物贸易减税计划正式启动，中国和文莱、印度尼西亚、马来西亚、缅甸、新加坡和泰国等东盟六国相互实施自由贸易区协定税率。
我国货物出口到上述国家，凡符合中国-东盟自由贸易区优惠关税的有关规定，只要签发了FORME证书，相当于有了一个“经济护照”，该批货物就能够享受更优惠的进口国关税待遇，平均减税幅度可达5%。
我国的FORM E由国家出入境检验检疫局签发。

实施普惠制必须遵循三个原则，即：1、非歧视性原则NON-DISCRIMINATION即把原先称作落后国家、不发达国家或新兴国家统称为“发展中国家”DEVELOPING COUNTRIES，亦称“受惠国”BENEFICIARY COUNTRIES;而把过去称作的先进国家或工业化国家一律成为“发达国家”ADVANCED COUNTRIES或“给惠国”PREFERENTIAL GIVING COUNTRIES。
2、惠遍原则GENERALIZED即对发达国家由发展中国家所进口的初级产品、半成品及商品实行普遍优惠制，给予普遍的、无例外的、不厚此薄彼的、一视同仁的优惠待遇。
3、非互惠原则NON-RECIPROCITY即是发达国家给予发展中国家或地区的普遍优惠，而不要求发展中国家给予发达国家提供反响优惠。
