进口锅炉压力容器报检条件审核表

备注（Note）:

1、此表一式三份，销售商（或报检单位）填写后携带合同草本及下述相关文件，应在合约正式签订的十五天前报江苏省特种设备安全监督检验研究院（简称JSSEI）进行报检条件审核，或者报检条件审核的时间必须满足JSSEI办理出国检验手续的时间要求（通常，JSSEI收到设备制造商的邀请信后，需45天才能办理完政府的出国批文、护照和签证的申请）。JSSEI审核提出意见后留存一份，销售商、用户，或中方签约单位各一份。

相关文件为：设备制造厂的有效的资格证书复印件（SQL证书或中华人民共和国特种设备制造许可证、或中华人民共和国锅炉压力容器制造许可证），合同技术附件和数据表等。

The review form will be in triplet. The seller should send the review form together with the contract protocol and the following relevant documents to JSSEI for reviewing of the contract terms and conditions at least 15 effective workdays before signing the formal contract, or the time applying for the contract terms & conditions review must meet the requirement that JSSEI can arrange the offshore inspection in time (generally, the procedure for the relevant authority permission and passport & visa application will take 45 days after receiving manufacturer’s letter of invitation). After review, one form will be kept in JSSEI, one for the seller, one for the user, and the rest one for China party.

The relevant documents are: The valid manufacturer’s certificate copy (Safety & Quality License, Manufacturing License of Special Equipment of P. R. China, or Manufacturing License of Boilers and Pressure Vessels of P. R. China), the contract technical attachments and data sheet etc..

2、介质参数是指蒸发量或容积、压力、温度。

The parameters mean the evaporation capacity or the volume, pressure, and temperature.

3、配套设备：

锅炉：燃用何种燃料的燃烧器、水处理设备、泵、风机、除氧器、加热器、安全阀、水位计等；

压力容器：爆破片或安全阀、液位计等。

Boiler’s appurtenance& supplementary equipment: burners which will be suitable for the fuels of the boilers, water treatment devices, pumps, blowers, deaerators, heaters, safety devices, water gages;

Pressure Vessel’s appurtenance and accessories: bursting discs, or safety valves, liquid level gages.

5、提供国家质量监督检验检疫总局锅炉压力容器安全监督局批文。

The seller should provide JSSEI with the written approval issued by Boiler & Pressure Vessel Safety Supervision Bureau of General Administration of Quality Supervision, Inspection, and Quarantine of the People’s Repub lic of China.
