男性型脱发患者头皮雄激素受体表达的研究

[摘要]目的:了解男性型脱发患者头皮雄激素受体(androgen receptor,ar)表达情况。方法:对来自10例男性aga患者和3例正常人的头皮,采用免疫组化的方法检测男性型脱发患者头皮不同区域ar的表达。结果:患者不同部位总的ar表达比较,均有明显差异(p0.05),但均明显强于正常对照组(p0.05)。结论:男性型脱发患者不同区域头皮ar的表达是有区别的;毛囊隆突部和漏斗部,交界区及脱发区ar表达明显增强,可能是男性型脱发的重要原因之一。

affiliated hospital of sun yat-san university;5.department of dermatology,guangzhou the eighth municipal people′s hospital 6.guangdong yuexiu plastic surgery hospital) abstract: objective to investigate ar (androgen receptor)expression in male pattern baldness. methods obtain follicles from different scalp areas(occipital area of non-hair loss,hair loss transition and hair loss zone area) of 10 male aga(androgenetic alopecia) patients and 3 normal scalp. use immunohistochemistry method to study ar expressions in hair follicles in different regions. results in the hair follicle bulge and infundibulum,ar expressions in hair loss area and the transitional area are obviously stronger than those in the occipital and the normal control group (p0.05) in occipital area,hair loss area and the transitional area, but there are significant differences compared to normal control group (p0.05). conclusion the ar expressions are different between different regions. in the hair follicle bulge and infundibulum, ar expressions in hair loss area and the transitional area are obviously stronger than those in the occipital. this may be one of the key factors leading to male pattern hair loss.

雄激素和遗传因素在雄激素源性脱发(aga)发病过程起着重要的作用[1]。雄激素的生物活性是由细胞内的雄激素受体所介导,雄激素受体(androgen receptor ar)存在于表皮、毛囊角质细胞、皮脂腺和汗腺等细胞和组织中。既往的研究发现脱发区与非脱发区真皮乳头细胞上ar表达是有区别的[2],但并没有阐明aga患者其它部位ar受体表达以及与正常人对照是否有差异。笔者推测在aga 患者中,脱发区不同部位的ar表达是有区别的,这在脱发的过程中可能起重要的作用。

本研究对10例男性aga患者不同区域(枕部、脱发过渡区和脱发区)的头皮,采用免疫组化的方法研究ar表达情况,并与年龄匹配的男性正常头皮相比较,现将结果报道如下。

1 资料和方法

1.1 临床资料

1.1.1 受试者纳入标准

1.1.1.1 aga组:经临床确诊的男性aga患者,年龄18~50岁,近3个月未系统使用抗雄激素药物和局部使用任何育发类产品或药物治疗,头皮正常完整,无细菌、真菌等感染性疾病及皮炎湿疹等炎症性皮肤病。

1.1.1.2 对照组:年龄在18~50岁,均为男性,本人及家族均无aga病史;近3个月未系统使用性激素药物,头发生长正常,头
