• 2.原发性中枢神经系统淋巴瘤(primary central nervous system lymphoma,PCNSL): 常见于免疫缺陷者,病程进展快。临床 表现缺乏特异性,常以血管周围间隙为中心向外 浸润生长。MRI上见PCNSL多累及脑实质,特别 是额顶叶和胼胝体部,病灶多发,常沿中线分布, 增强见均匀强化。 MRS有助于PCNSL诊断,常见Cho/NAA 比值明显升高。PCNSL对类固醇激素治疗不敏感, 常可见复发。因此对于类固醇激素治疗效果不佳 的疑似CLIPPERS患者需考虑PCNSL可能。确诊 亦需病理结果。

概述
• 类固醇激素反应性慢性淋巴细胞性炎症伴脑桥血 管周围强化症(chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids,CLIPPERS)是指在脑桥、 中脑及小脑血管周围以淋巴细胞浸润为主、对类 固醇激素治疗有效的慢性炎性疾病。
• 该综合征由Pittock等于2010年首次报道,之后报 道陆续增多,到目前为止国外共报道40例。2013 年国内首次报道1例CLIPPERS。
CLIPPERS的神经病理学特点
• 约半数患者进行了脑活体组织检查,检查部位有小脑、基 底节、丘脑及脑桥等。
• 所有检查结果均显示脑白质、小动脉和小静脉血管周围有 明显淋巴细胞炎性浸润(包括小淋巴细胞、巨噬细胞和反 应性小胶质细胞),偶可见类似肉芽肿表现,但却没有明 显的周围淋巴细胞聚集或组织坏死。
