Equipment Lifting Safety Method Statement

A、编制说明

Preparation Instructions

本方案编制根据目前乐高玩具中国嘉兴工厂项目现场实际情况编制，方案具体编制如下：

This scheme establishment according to current Novartis Shanghai campus phase 1 C11-2 the scene actual situation, the plan establishment is as follows:

1、A、吊装方案报管理公司、监理、业主方审定。

Hoisting plan should be submitted to MC，Supervision，GC for examination and approval.

B、方案批准后，施工人员必须严格执行方案。

After approval construction persons must strictly implement the scheme.

2、为确保吊装任务顺利完成特制订施工方案,必须严格实行：

In order to ensure smoothly completing of lifting work to formulate this construction scheme, and strictly carrying out .

1）必须召开吊装启动会议，协调各单位步调一致。

Must be hoisting launch meeting, and coordinate all in same line.

2）确定吊装起吊时间。

To confirm lifting operation time.

3）确定道路封锁区域和时间。

To confirm road closed area and time.

4）场地要求：

Site requirement:

A、根据目前现场工地看察情况，本工程使用50t汽车起重机一台吊装就位，要求车辆停放位置一个。

According to current site that was made in situation , GROVE GMK-7450-450 mobile crane will be used in this project for the equipments lifting and installation, and crane sitting position.

B、工地现场必须清除堆放物及平整压实吊装场地。

Construction site to be cleaned to the stacking , and leveling and ramming ground in work place.

C、吊装施工期间同一场地内必须停止其他施工,设置安全警戒区域标志，其他非施工人员禁止在吊装工作区间内逗留。

All the construction work will be stopped within lifting area except lifting operation work, and segregation around lifting area, and put signage , and all the persons nothing with lifting work to keep out of the lifting area.

D、吊装环境是否有有毒气体、高压电线、地下管道、电缆沟等，派专职人员做监控处理。

Checking around lifting area environment if there are toxic gas, high voltage cable, underground pipe, cable trench etc. All to be done by professional person.

E、现场备有卫生急救包及消火器、黄沙等，保持水源畅通。

First-aid packet, fire fighting extinguisher , yellow sand etc to be prepared on the site and keep sufficient water.

F、六级以上台风禁止作业。

To forbid operation work if wind speed is over 10.8 m/s.

C、本施工主要包括以下内容：

The construction is as follows:

1. Large-sized truck crane, flatbed truck for conveyance taking possession of the site, assembly and test run.大型汽车吊、运输平板车的进场、组车和试车；

2. Equipment transportation设备运输；

3. Lifting and locating of large-scaled equipment大型设备的吊装就位；

4. Lifting and locating of auxiliary equipment附属设备的吊装就位；
