酒石酸布托啡诺镇静、镇痛效果优于盐酸吗啡,常规量的枸橼芬太尼与盐酸曲马多镇痛效果较盐酸吗啡和酒石酸布托啡诺差,且盐酸曲马多恶心呕吐发生率和盐酸吗啡相同,甚至还要高,成为多数产科医生和产妇不愿接受的原因之一。
酒石酸布托啡诺属于混合型阿片受体激动-拮抗药,主要激动κ和δ受体,并拮抗μ受体,该药能有效地减轻中度及重度疼痛,镇痛效价约为盐酸吗啡的5~8倍,同时具有镇静作用,很少出现欣快及不愉快的感觉[1]。
通常认为,恶心呕吐、皮肤瘙痒是由于盐酸吗啡刺激μ受体而引起;μ受体兴奋可以导致胃肠排空时间延长,胃肠平滑肌张力增高,从而增加恶心呕吐的发生率。
而盐酸布托啡诺是吗啡喃的衍生物,具有镇痛效应强、镇痛时间久、胃肠道不良反应少、对呼吸抑制作用小、药物依赖性低等特点[2],是剖宫产术后连续硬膜外镇痛的较好的选择。

All Rights Reserved.态错失恐惧在AM I病人基本需要满足与幸福感之间中介效应,从而为改善AM I病人的心理状态,提升其幸福感的干预措施制定提供参考依据㊂1对象与方法1.1对象采用便利抽样的方法选取本院2021年5月 2022年1月收治的AM I病人为调查对象㊂纳入标准:1)年龄ȡ18岁;2)符合‘内科学“第7版关于心肌梗死的诊断标准[13];3)能够独立填写问卷;4)签订知情同意书㊂排除标准:1)合并脑卒中㊁肝肾功能障碍㊁恶性肿瘤等严重疾病;2)伴有精神疾病或智力障碍;3)近期(2~3个月)有精神类药物服用史或吸毒史㊂调查人数应为纳入调查项目的10~15倍[14],本研究共15个项目,应调查150~225例,考虑到10%~30%失访率,样本量至少165例,本研究样本量为292例㊂1.2方法1.2.1调查工具1)基本情况问卷㊂采用本课题组自制的问卷调查AM I病人性别㊁年龄㊁文化程度㊁婚姻状况㊁是否伴有其他慢性疾病(糖尿病㊁高血压㊁慢性阻塞性肺疾病或其他慢性病)㊁婚姻状态㊁是否独居㊁子女人数㊁家庭人均月收入㊁有无固定职业和自评睡眠质量等㊂2)特质-状态错失恐惧量表(T r a i t-S t a t eF e a ro fM i s s i n g O u t S c a l e,T-SF o MO S)㊂T-SF o MO S由W e g m a n n等[15]编制,用于评估个体特质性与状态性错失恐惧状况,后由肖曼曼等[16]于2019年汉化翻译,得到中文版量表,其C r o n b a c h'sα系数为0.824,包括特质性错失恐惧(5个条目)和状态性错失恐惧(7个条目)2个维度,共12个条目,每个条目选择L i k e r t5点计分,1~5分依次为 非常不同意 到 非常同意 ,总分12~60分,得分越高,受访AM I病人特质及状态错失恐惧程度越高㊂3)基本心理需要满足量表(t h eB a s i cP s y c h o l o g i c a l N e e d sS a t i s f a c t i o n S c a l e,B P N S S)㊂B P N S S由D e c i 等[17]于2001年编制,用于评估个人生活中的心理满足需求程度,后由谢东杰等[18]于2009年进行中文翻译和修改,中文版量表内部一致性C r o n b a c h'sα系数为0.876,包括能力需要(6个项目)㊁关系需要(8个条目)和自主需要(7个条目)3个维度,共21个条目,每个条目选择L i k e r t7点计分,1~7分依次为 完全不同意 到 完全同意 ,总分21~147分,得分越高,病人心理满足需要程度越高㊂4)纽芬兰大学幸福感量表(M e m o r i a l U n i v e r s i t y o f N e w-f o u n d l a n d S c a l e o f H a p p i n e s s,MU N S H)㊂MU N S H由K o z m a等[19]编制,用于反映个体生活满意度指数和情感平衡状态,后由刘仁刚等[20]进行中文翻译及修订,中文版量表C r o n b a c h'sα系数为0.815,内容效度为0.785,包括正性情感(p o s i t i v ea f f e c t i o n,P A)㊁负性情感(n e g a t i v e a f f e c t i o n,N A)㊁正性体验(p o s i t i v ee x p e r i e n c e,P E)和负性体验(n e g a t i v e e x p e r i e n c e,N E)4个维度,其中P A 和N A均为5个条目,P E和N E均为7个条目,共24个条目,每个条目0~2分,回答 是 计2分,回答 不知道 计1分,回答 否 计0分,总分=P A得分-N A 得分+P E得分-N E得分+常数,常数为24,总分0~48分,得分越高,病人幸福感越高㊂1.2.2调查方法本研究调查开始前1周,按照纳入和排除标准选择50例AM I病人进行预调查,由病人自行填写,时间为5~8m i n,发放问卷50份,收回有效问卷50份㊂T-SF o MO S内部一致性C r o n b a c h'sα系数为0.752, B P N S S内部一致性C r o n b a c h'sα系数为0.898, MU N S H内部一致性C r o n b a c h'sα系数为0.824㊂正式调查地点为医护值班室,由2名课题组成员进行调查,其中1名采取统一规范语言向受访AM I病人讲解本次调查目的㊁意义和过程,另1名发放问卷,由病人自行填写,规定填写时间为8m i n,填写结束当场收回,不记录姓名㊂2名调查者复核收回问卷有无漏填,共发放问卷292份,收回292份,得到有效问卷284份,有效问卷率97.26%,采取双人录入方式将问卷数据整理输入电脑㊂1.2.3统计学方法采用S P S S24.0及其插件P r o c e s s3.0对本研究数据进行统计分析,定性资料采用例数㊁百分比(%)描述,符合正态分布的定量资料以均数ʃ标准差(xʃs)描述,采用H a r m a n单因子检验法检验所有因子共同方法偏差效应,B o o s t r a p法检验结构方程模型拟合度,检验水准α=0.05㊂2结果2.1 AM I病人一般资料284例AM I病人,年龄34~79(52.75ʃ14.72)岁,基本情况具体见表1㊂㊃6952㊃C H I N E S EE V I D E N C E-B A S E D N U R S I N GJ u l y,2023V o l.9N o.14Copyright©博看网. All Rights Reserved.分,相比于总分25分,处于中等水平㊁状态错失恐惧得分为(27.47ʃ8.66)分,相比于总分35分,处于中等水平㊂本研究结果表明,AM I病人具有一定错失恐惧㊂这是因为特质与状态错失恐惧是指因担心错过有益的信息或活动,从而希望时刻保持对外界信息及活动的关注[29]㊂随着互联网时代的到来,人们获取信息的渠道越来越多样化,投入社交媒体上的时间也更多㊂而此次调查对象中,中青年㊁已婚㊁与家人同住㊁有子女人群占比较高,在社会中承担的角色较多,对信息的需求较高㊂而AM I发病较急,病人难以短时间内适应疾病状态,即使出院,AM I病人也因长期服药需要持续获取疾病相关信息㊂此次调查对象中,硕士以下人群占92.25%,说明此次调查对象整体学历层次较低,获取信息的能力较弱,更易因担心错过或急于获取疾病相关信息处于焦虑状态㊂因此,医护人员应结合病人的人口学特点,对于中青年㊁已婚㊁学历层次较低等AM I 病人适当提高健康宣教的频次或丰富健康宣教的内容以降低该类病人的特质与状态错失恐惧水平㊂3.2 AM I病人基本心理需要满足与幸福感呈正相关本次调查结果显示,AM I的基本心理需要满足与幸福感呈正相关(P<0.001),此研究结果与其他研究学者的调查结果一致[30]㊂当个体在社会中承担的角色㊁接受的支持更多,就会感受到更高的归属感以及胜任感,从而产生更高的基本需要[31]㊂随着社会压力增加,AM I病人呈现年轻化态势㊂中青年群体是社会的中流砥柱㊂因此也承担更多角色,需求更多㊂当基本心理需要满足状况良好时,将引起病人认知及情感层面的积极变化,从而提升其幸福感水平㊂因此,医护人员可以通过关注病人的心理需要,尊重病人自身兴趣(自主感)如鼓励病人参与治疗方案的制定,增强社会支持(归属感)如家庭干预㊁同伴教育等方式,提高AM I病人的幸福感㊂3.3 AM I病人特质与状态错失恐惧与心理需要满足㊁幸福感之间呈负相关本次调查结果显示,AM I病人特质错失恐惧㊁状态错失恐惧与心理需要满足㊁幸福感呈负相关(P< 0.001),表明高特质与状态错失恐惧会降低病人的心理需要满足感以及幸福感水平㊂这可能因为当前经皮冠状动脉介入术和溶栓治疗是AM I病人主要的治疗手段[32],但是术后AM I病人须接受长期的康复治疗,会对日常生活造成一定影响,使其更渴望了解他人的有益经历并且担心错过工作生活中的有益信息,一旦错过某些重要内容,病人会产生不同程度的焦虑情绪,从而降低其心理满足感及幸福感水平㊂因此,医护人员应重点关注AM I病人心理焦虑程度,采取积极干预手段缓解其因疾病造成其无法正常获取信息而产生的焦虑感,达到改善其心理满足感和增强其幸福感的目的㊂3.4 AM I病人特质与状态错失恐惧的中介效应本研究中介效应分析结果显示,特质错失恐惧和状态错失恐惧均在基本心理需要满足与幸福感之间存在链式中介效应㊂结果表明,基本心理需要可通过状态错失恐惧对幸福感产生影响,与李莉[33]研究结果一致㊂国外学者认为,若个体心理需求满足感缺失会导致自我监管不足从而产生错失焦虑[10]㊂在互联网时代,社交媒体丰富了人们与外界的连接渠道,心理需求尚未得到满足的个体会倾向于借助手机㊁电脑等移动设备寻求外界的帮助与关注㊂AM I病人发病较急,尚未完全适应疾病的状态,除病人的身份外,还承担多种社会角色,需求较多,若基本心理需求得不到满足,无法感受到一定的支持及价值时,可能更加关注外界的信息,并且期待了解他人的看法及有益经历,从而经常下意识地查看自己的社交媒体,渴望外界的肯定,加之长期的康复治疗要求病人对外界的消息保持关注,在追求获取他人信息的同时,个体可能会忽视当下的乐趣,进而处于焦虑状态,降低幸福感㊂因此,医护人员应该关注AM I病人的特质与状态错失恐惧状态,建议采取团体心理辅导㊁线上线下结合的健康宣教㊁延续护理等多种方式尽量满足病人的心理需要,降低病人的特质与状态错失恐惧,从而提升病人的幸福感㊂4小结综上所述,本研究结果表明,特质与状态错失恐惧在AM I病人心理需要满足和幸福感之间有中介作用㊂因此,医护人员应注重病人的相关健康宣教,提升其自主需要及胜任需要,降低他们特质与状态错失恐惧,从而增加其幸福感㊂由于本研究仅调查本院的284例AM I病人,区域单一且不具有代表性㊂今后可扩大样本量,针对不同地区㊁不同级别的医院进行调查研究㊂也会根据此次调查的结果,制定针对性的干预策略,以提高AM I病人的幸福感㊂参考文献:[1]国家心血管病中心.‘中国心血管健康与疾病报告“2020[J].心肺血管病杂志,2021,40(10):1005-1009.[2]何源,徐东杰.‘2021E S C急慢性心力衰竭诊断和治疗指南“中的新机制㊁新研究㊁新推荐[J].心脑血管病防治,2022,22(1):4-7.[3] P O G O S O V A N,S A N E R H,P E D E R S E N SS,e ta l.P s y c h o s o c i a la s p e c t s i n c a r d i a c r e h ab i l i t a t i o n:f r o mt h e o r y t o p r ac t i c e.A p o s i t i o np a p e r f r o m t h e C a r d i a c R e h a b i l i t a t i o n S e c t i o n o ft h e E u r o p e a n㊃9952㊃循证护理2023年7月第9卷第14期(总第106期)Copyright©博看网.
