HLA主动免疫治疗原因不明反复自然流产92例分析

反复性自然流产是指临床上连续2次或2次以上的自然流产，是一种常见的妊娠并发症，其中找不到明确病因者称为原因不明反复自然流产（unexplained recurrent spontaneous abortion，URSA）[1]，临床上反复性自然流产患者中有40%～80%为URSA。目前认为URSA多数与免疫因素有关，HLA-DR抗体能抑制母体识别父源性HLA抗原，对胎儿有明确的保护作用。研究发现，部分流产患者血清中缺乏HLA-DR抗体[2]。本文对近年来我院收治的HLA-DR抗体阴性URSA患者进行淋巴细胞主动免疫治疗，现报道如下。

1资料与方法

1.1一般资料

2002年4月～2009年8月在我院诊断为URSA的患者共92例；年龄22～41岁，平均31岁；不明原因自然流产2～6次，平均3.5次；流产时孕周为5～10周。患者丈夫均无经血液传播性传染病。1.2实验室检查

各项检查（包括染色体、精液常规、抗子宫内膜抗体、抗卵巢抗体、抗精子抗体、TORCH、内分泌激素等）均正常。无生殖道解剖异常、无自身免疫及内分泌疾病。微淋巴细胞毒性实验阴性。

1.3治疗方法

对HLA-DR抗体阴性患者进行HLA主动免疫治疗。采集丈夫静脉血20mL，用肝素抗凝管收集血液样本，分离淋巴细胞后将样本浓缩至1.0～1.5mL，皮内注射至妻子前臂内侧，分6～8点注射。每2周治疗1次，3次为1个疗程，之后复查HLA-DR 抗体水平。根据HLA-DR抗体复查结果可加强免疫治疗1～2个疗程，直至HLA-DR抗体转阳后准备怀孕。确认宫内妊娠后加强免疫治疗1～2个疗程。

2结果

[摘要]目的观察HLA主动免疫治疗原因不明反复自然流产（URSA）的疗效。方法对92例URSA患者用丈夫淋巴细胞进行前臂内侧皮内注射，注射分6～8点进行，每2周治疗1次，3次为1个疗程。对于HLA-DR抗体复查阴性者加强免疫治疗1～2个疗程，确认宫内妊娠后加强免疫治疗1～2个疗程。结果经HLA主动免疫治疗1个疗程后HLA-DR抗体转阳者58例，转阳率为94%。经过2个疗程治疗后HLA-DR抗体转阳者21例，转阳率为78%。92例URSA患者最终成功妊娠85例，成功率达92%。结论HLA主动免疫疗法可有效提高HLA-DR抗体阴性URSA患者妊娠成功率，HLA-DR抗体检测可作为习惯性流产临床诊断和免疫治疗的指标之一。

[Abst r act]Obiect ive To assess the therapy effect of HLA active specific immunotherapy for unexplained recurrent spontaneous abortion （URSA）.Met hods The intradermal injection of their husbands'leukomonocyte was given to92cases of URSA once two weeks，with3times as a therapy course.one to two therapy courses of the strengthened immune treatment was given to those with negative results of HLA-DR an-tibody test and one to two therapy courses of the strengthened immune treatment was given to those whose intrauterine pregnancy was con-firmed.Result s In92cases，58cases showed HLA-DR antibody positive after one course of HLA active therapy，with the positive conversion rate of94%，and21cases showed the positive results of HLA-DR antibody after two course of HLA active therapy，and the positive conversion rate of78%，and85of92cases of URSA showed successful pregnancy after HLA active immunotherapy，with the achievement rate of92%. Conclusion HLA active specific immunotherapy may increase the pregnancy rate of URSA cases with negative HLA-DR antibody.HLA-DR antibody can be a useful marker for the diagnosis and immunotherapy of URSA.
