慢性心衰(CHF)是一种复杂临床综合征,亦称慢性心功能不全,是各类心脏病的严重结局之一。近年认识到CHF发生发展的基本机制是心室重塑(VR)。故CHF的药物治疗策略也从传统的“强心、利尿、扩血管”为主的治疗模式转变为拮抗神经内分泌激活、阻止心肌重塑为主的生物学治疗模式。笔者以往的研究[1]

【摘要】目的探讨参竹心康汤对慢性心力衰竭大鼠心室重构及心功能的影响。方法SD大鼠72只随机分为假手术组,模型组,卡托普利组,参竹心康汤低、中、高剂量组,每组12只。采用腹主动脉缩窄法制作心衰大鼠模型。于造模成功后,假手术组、模型组以蒸馏水10mL/kg灌胃;卡托普利组以卡托普利混悬液7.8mg/kg灌胃,低剂量组、中剂量组、高剂量组分别以中药煎液(含生药4.5g/mL)11、22、44g/kg灌胃,每日1次,连续8周。观察大鼠血流动力学及心功能参数,检测血浆血管紧张素Ⅱ(AngⅡ)和醛固酮(ALD)水平,心脏标本检测左室重量(LVW)及左室质量指数(LVMI)。结果模型组左室舒张末压(LVEDP)、血浆AngⅡ和ALD水平、LVW及LVMI明显高于假手术组,而平均动脉压(MAP)、左室收缩内压峰值(LVSP)、左室内压最大上升和下降速率(±dp/dt)均明显低于假手术组;卡托普利组、中剂量组、高剂量组MAP、LVSP、±dp/dt均明显高于模型组和低剂量组;而LVEDP、血浆AngⅡ和ALD水平、LVW与LVMI均明显低于模型组和低剂量组;低剂量组LVSP高于模型组,低剂量组LVEDP低于模型组。心率在各组间比较差异无统计学意义。结论参竹心康汤能改善慢性心力衰竭大鼠心室重构,保护心功能。

【Abstract】Objective:To observe the effect of Shenzhuxinkang Decoction on ventricular reconstruction and car-diac function in chronic heart failure rat.Methods:72SD rats were randomly divided into6groups averagely: sham operation group,model group,captopril group,and Shenzhuxinkang Decoction groups with low,medium and high dose(low dose group,medium dose group and high dose group).The sham operation group and model group were gavaged with distilled water with10ml/kg,captopril group with Captopril suspension with7.8mg/kg,and low dose group,medium dose group and high dose group respectively with decoction(containing crude drugs4.5g/ mL)with11g/kg,22g/kg,44g/kg.All groups were gavaged1time a day for8weeks.The parameters of haemo-dynamics and cardiac function were observed,the amount of plasmatic angiotensinⅡ(AngⅡ)and aldosterone (ALD)were tested,and the left ventricular weight(LVW)and left ventricular mass index(LVMI)were detected. Results:The model group had an apparently higher level in left ventricular end-diastolic pressure(LVEDP),plasmatic angiotensinⅡ(AngⅡ)and aldosterone(ALD),LVW and LVMI than sham operation group versus ob-vious lower level in mean arterial pressure(MAP),left ventricular contraction internal pressure peak(LVSP)and ±dp/dt than the sham operation group.Captopril group,medium dose group and high dose group were significantly higher in MAP,LVSP and±dp/dt whereas much lower in LVEDP,AngⅡ,ALD,LVW and LVMI than model group and SZXKDL group.The low dose group had a higher level in LVSP and lower level in LVEDP while com-pared with the model group.The heart rate difference was not statistically significant among groups.Conclusion: Shenzhuxinkang Decoction can improve the ventricular reconstruction and protect cardiac function in chronic heart failure rat.
