合众团体附加意外伤害医疗保险（2013修订）条款Provisions for Additional Accidental Injury Medical Insurance (2013 Revision) of UnionLife(合保发〔2013〕483号，2013年12月经保监会核准备案）(Union Life Issuance [2013] No. 483, which is approved by CIRC in Dec. 2013)在本条款中，本公司指合众人寿保险股份有限公司In this provision, the company refers to Union Life Insurance Co., Ltd.1. 投保人与本公司的合同1. Contract signed between the policy holder and the company1.1 投保范围1.1 Scope of Insurance凡在本附加保险合同（以下简称“本附加合同”）订立时对被保险人具有保险利益或经被保险人书面同意的机关、企业、事业单位和社会团体均可作为投保人。

When signing this Additional Insurance Contract (hereinafter referred to as “Additional Contract”), policy holder can be the one who links with the insured in terms of insurance interest or government units, enterprises, public institutions and social organizations which are permitted by the insured in written.凡符合本公司承保条件的人均叫作为被保险人。

All those complying with the insurance conditions can be considered as the insured.1.2 合同构成1.2 Contract Formation本附加保险合同需由主保险合同(以下简称“主合同”）投保人提出申请,经本公司同意而订立。

This Additional Contract shall be signed once being applied by the insured set in the Master Insurance Contract (hereinafter referred to as “Master Contract”)and being agreed by the company.主合同所附条款、投保书、合法有效的声明、批注、附贴批单、其他书面协议，凡与本附加合同相关者，都是本附加合同的构成部分。

若主合冋与本附加合同的条款互有冲突，则以本附加合同的条款为准。

This Additional Contract includes all provisions set in the Master Contract, proposal form, legal and valid declaration, notes, approval orders attached to this Additional Contract, other written agreements and parties related to this Additional Contract. If there is a conflict between this Additional Contract and Master Contract, Additional Contract shall be prevailed.1.3 投保人解除合同的手续及风险1.3 Formalities and Risks of Terminating the Contract by Policy Holder如投保人申请解除本附加合同，请填写解除合同申请书并向公司提供如下资料：If the policy holder applies to terminate this Additional Contract, it is necessary to fill the application form for contract termination and submit documents listed here below:(1) 保险合同:(1) Insurance Contract(2) 保险费收据。

(2) Insurance fee receipts自本公司收到解除合同申请书时起，本附加合同终止，本公司自收到解除合同申请书之日起30日内向投保人退还保险单的现金价值（见释义6.1）.This Additional Contract will be terminated from the day when the company receives the application of terminating contract. Then the company shall refund the cash value (refer to explanation 6.1) of the insurance policy to the policy holder within 30 days upon receiving the application.2. 本公司提供的保障2. Guarantees Provided by the Company2.1 保险金额本附加合同被保险人的保险金额由投保人与本公司约定并在保险单中载明。

2.1 Insured Amount. The insured amount for the insured in this Additional Contract shall be agreed by the policy holder and the company, which shall be clearly described in the insurance policy.2.2 保险期间本附加合同的保险期间同主合同。

2.2 Insurance Period. The insurance period of this Additional Contract is the same as that of the Master Contract.2.3 保险责任在本附加合同保险期间内，本公司承担下列保险贵任:2.3 Insured Liability. During the insurance period, the following liabilities are to be undertaken by the company:若在理赔时，被保险人己从社会基本医疗保险、公费医疗或其他途径（包括：工作单位以及包含本公司在内的任何商业保险机构费用补偿型产品等）获得补偿，本公司将在扣除前述补偿后，按剩余的部分给付意外伤害医疗保险金。

At the time of claim, if the insured has obtained compensation from social basic medical insurance, free medical service or others (including: compensation provided by his or her company or the company or other commercial insurance organizations), the company will pay the insurance benefit for accidental injury to the insured after deducting the compensation aforementioned, whose calculation is based on the balance of medical expense.若在理赔时，被保险人未从社会基本医疗保险、公费医疗和其他途径（包括：工作单位以及包含本公司在内的任何商业保险机构费用补偿型产品等）获得补偿，本公司将在扣除100元免赔额后，按剩余的部分给付意外伤害医疗保险金。

At the time of claim, if the insured does not obtain compensation from social basic medical insurance, free medical service or others (including: compensation provided by his or her company or the company or other commercial insurance organizations), the company will pay the insurance benefit for accidental injury to the insured after deducting the deductible (RMB 100 Yuan), whose calculation is based on the balance of medical expense.2.4保险责任的免除2.4 Exemption of Insurance Liabilities被保险人因以下情形之一发生医疗费用的，本公司不承担给付保险金的责任：If there is one of the following situations that the insured pays medical expense, the company will not undertake the responsibility to pay insurance benefit to the insured:(1)投保人对被保险人的故意杀害、故意伤害；(1) The policy holder kills or injures the insured on purpose;(2)被保险人故意犯罪或者抗拒依法采取的刑事强制措施；(2) The insured has a crime on purpose or the insured resists to the criminal compulsory measures which are legal;(3)被保险人主动吸食或注射毒品（见释义6. 4);(3) The insured actively takes drugs or injects drugs (refer to explanation 6.4);(4)被保险人酒后驾驶（见释义6. 5)，无合法有效驾驶证驾驶（见释义6. 6)或驾驶无有效行驶证（见释义6. 7)的机动车；(4) The insured drives after taking wine (refer to explanation 6.5), or the insured does not have a legal and valid license (refer to explanation 6.6) or the insured drives a motor vehicle without having a valid driving license (refer to explanation 6.7));(5)战争、军事冲突、暴乱或武装叛乱；(5) The insured suffers accidental injuries because of a war, military conflict, rebellion or armed rebellion;(6)核爆炸、核辐射或核污染；(6) The insured suffers accidental injuries because of nuclear explosion, nuclear radiation or nuclear contamination;。
