Vendor or the third party appointed by vendor hereby affirms that the promoter holding this Checklist are lawfully employed and appointed by vendor or the third party. Vendor or the third party appointed by vendor hereby authorizes the promoter to, in vendor’s name and on vendor’s behalf, engage in work related to merchandise promotion in Wal-Mart stores, and his primary work is to promote sales and serve customers. Meanwhile, the promoter can make cleanup and replenishment within the area where the shelves containing the merchandise and the special cabinets are located. Vendor or the third party appointed by vendor shall be responsible for any and all activities conducted by and all damages suffered by the promoter during the time of promotion.

Vendor or the third party appointed by vendor guarantee the truth of above statement and bear full responsibility for consequences resulted from any false information above.

供应商或者是供应商授权的第三方在此确认，持本表格前往商场的促销员是该供应商或供应商授权的第三方合法派出的该公司员工，供应商或者是供应商授权的第三方授权该促销员在沃尔玛商场内，以供应商的名义，代表供应商开展与商品促销相关的工作，主要职责是促进销售和服务顾客，同时对商品的货架和专柜进行区域整理和补货。供应商或者是供应商授权的第三方应对其促销员在商品促销期间的一切行为、所遭受的伤害依法承担责任。

供应商或者是供应商授权的第三方承诺对上述陈述的真实性负责，并愿意承担因上述资料虚假所可能导致的一切责任。
