改良Henry入路结合LCP钢板治疗不稳定型桡骨远端骨折临床疗效分析

摘 要：研究改良的 Henry 入路结合 LCP 钢板治疗不稳定型桡骨远端骨折的临床疗效;筛选了本科从 2016 年- 2018 年收 住入院的 68 个不稳定型桡骨远端骨折患者的病例，且均进行随访，运用随机分组把患者划分为试验组和对照组，每组例数相 同，试验组运用改良的 Henry 入路结合 LCP 钢板治疗;对照组运用掌直接入路结合 LCP 钢板治疗，然后进行研究分析骨折暴露 时间还有正中神经刺激率和 Gartland- Werley 评分评定临床的治疗效果；数据显示：试验组在骨折暴露时间低于对照组，且两 组比较差异具有统计学意义（P ＜ 0.01）；试验组正中神经刺激率低于对照组，两组比较差异具有统计学意义（P ＜ 0.01）；试验组 在旋前方肌修补率上明显高于对照组，且两组比较差异具有统计学意义（P ＜ 0.01）；两组在 Gartland-Werley 评分上的统计学基 本没有差异；结论：改良 Henry 入路结合 LCP 钢板治疗不稳定型桡骨远端骨折具有操作简单、旋前方肌修复率高、正中神经刺 激率低的优点。

Abstr act：Objective To investigate the clinical effect of modified Henry approach combined with LCP plate in the treatment of unstable distal radius fractures. Sixty- eight cases with unstable distal radius admitted to our department from January 2016 to January 2018 were selected. All cases were followed up. Patients were divided into experimental group and control group by random number method, with 34 cases in each group. The experimental group was treated with modified Henry approach combined with LCP plate. The control group was treated with direct palmline approach combined with LCP plate. Clinical efficacy was evaluated by observing the exposure time of fracture, median nerve stimulation rate, anterior rotatory muscle repair rate and gartland- werley score. Results The duration of fracture exposure in the experimental group was significantly lower than that in the control group, and the difference between the two groups was statistically significant （P < 0.01）. The median nerve stimulation rate in the experimental group was lower than that in the control group, and the difference between the two groups was statistically significant（P < 0.01）. The repairing rate of anterior rotator cuff in the experimental group was significantly higher than that in the control group, and the difference between the two groups was statistically significant （P < 0.01）. There was no significant difference in gartland- werley score between the two groups. Conclusion The modified Henry approach combined with LCP plate in the treatment of unstable distal radius fractures has many advantages including simple operation, high repairing rate of anterior rotator cuff and low median nerve stimulation rate.
