填表说明：

为了安全作业，对所有的问题均应肯定地做出回答，可在相应的方格内清楚地标上“V”记号。如果不能作肯定的回答，则应说明理由，并就所应采取的相应的措施在船舶与油码头之间达成协议。对认为不适用的任何问题，就在备注栏中注明。

“船舶”和“油码头”栏目下的方格，是供双方各自填写检查结果的。

在“代码”栏目中标有字母A、P或Ｒ者，分别表示如下意义：

A——任何议定的程序和协议，均应以书面方式写明在本检查表的“备注”栏中，或者互相认可的其他表式中，无论哪一种方式，都应要求双方签字。

P——在否定回答的情况下，未经港口主管当局的准许，不可进行作业。

R——表示对不超出声明中时间间隔需要重复检查的项目。
