间质性肺病合并肺癌的研究进展

摘要

由于间质性肺病及肺癌在病理学机制存在一定的相关性，且间质性肺病可能表达某些肿瘤因子，使得临床上间质性肺病患者的肺癌发病率较普通人群显著升高。但其临床及影像学的表现无特异性，可能导致肺癌的漏诊及误诊，且目前肺癌的治疗方法均可能诱发间质性肺病的急性加重，使得间质性肺病合并肺癌患者的生存时间明显缩短，生存质量明显下降。近年来对于间质性肺病与肺癌之间的发病机理、肿瘤标志物、以及间质性肺病合并肺癌的诊断及治疗、间质性肺病的筛查等方面的研究有所进展，临床上提高了该类患者的生存质量。本文着重从间质性肺病合并肺癌的流行病学、发病机制、肿瘤标志物、临床诊断及治疗等方面，综述了近年来间质性肺病合并肺癌的研究进展。

THE PROGRESS OF INTERSTITIAL LUNG DISEASE

WITH LUNG CANCER

ABSTRACT

The incidence lung cancer in the interstitial lung disease patients is higher than common patients. It’s easy to missed diagnosis and misdiagnosis due to its nonspecific clinical and imaging findings, and the treatment for lung cancer could induce interstitial lung disease acute exacerbation. The survival time and quality of interstitial lung disease and lung cancer patient is obviously decreased. In recent years, with studying the diagnosis

and treatment of intestinal lung disease with lung cancer, the survival quality of patients has been developed. This paper, from epidemiology, mechanisms, tumor biomarkers, clinical diagnosis and treatment, provides an overview of recent research progress on intestinal lung disease with lung cancer.

间质性肺病合并肺癌的研究进展

前言

间质性肺病（Interstitial lung disease，ILD）是一大类肺部疾病的统称，不仅累及肺间质，同样也累及肺实质，虽然ILD包含许多特定的肺部疾病，但它们均具有相似的临床、影像学及病理特征。其基本病理表现为弥漫性肺实质、肺泡炎及间质纤维化，以限制性通气障碍、弥散功能降低、低氧血症和活动性呼吸困难，X线胸片示双肺网状、结节状或磨玻璃样阴影为临床表现。主要包括已知病因的间质性肺病、特发性间质性肺炎（Idiopathic interstitial pneumonia，IIP）、肉芽肿性肺病和其他间质性肺病，其中IIP是一组病因尚不明确的间质性肺病，其分类几经变更，不断修改，一直未达到共识，各国命名也不同。2013年美国胸科协会及欧洲呼吸协会对特发性间质性肺炎的分类，特发性间质性肺病根据组织病理学特点又分为常见特发性间质性肺炎：特发性间质性肺纤维化（Idiopathic pulmonary ?brosis，IPF）、特发性非特异性间质性肺炎（Idiopathic nonspeci?c interstitial pneumonia，INSIP）、呼吸性细支气管炎相关间质性肺病（Respiratory bronchiolitis–interstitial lung disease，RB-ILD）、脱屑性间质性肺炎（Desquamative interstitial pneumonia，DIP）、隐源性机化性肺炎（Cryptogenic organizing pneumonia，COP）、急性间质性肺炎（Acute interstitial pneumonia，AIP）；罕见特发性间质性肺炎：特发性淋巴细胞间质性肺炎（Idiopathic lymphoid interstitial pneumonia，LIP）、特发性胸膜肺弹力纤维增生症（Idiopathic pleuroparenchymal ?broelastosis，PPFE）；以及未
