• MYC is activated in aggressive lymphomas derived from both germinal center and terminally differentiated B-cells. • MYC is activated by genetic alteration and transcriptional mechanisms. • MYC genetic and protein alterations are frequently associated with activations of other oncogenes conferring a worse prognosis • Genetic and IHC double hits are associated with poor prognosis but may not have the same clinical significance • Screening with IHC followed by FISH analysis may be a strategy to facillitate identification of patients with a very aggressive disease.
