1.试述急性化脓性鼻窦炎的发生与鼻窦解剖特点的关系。
答案:本病与鼻窦的解剖特点有关:
①窦口小,鼻道狭窄而曲折,易于阻塞,引起鼻窦通气引流障碍。

②鼻窦粘膜与鼻腔粘膜相连续,鼻腔粘膜炎症常累及额窦和上颌窦粘膜。

现代观点认为前组筛窦炎是

④各窦自身特点及窦口的位置:上颌窦最大,但窦口高,但在中鼻道的位置最后、最低,受累机会最多;筛窦为蜂房状结构,不利于引流,感染机会相对较多;此外,上颌窦和筛窦发育最早,故儿童期即可罹患;额窦虽位置高、窦口低,但因毗邻前组筛窦,故亦易受累;蝶窦位于各窦之后上,且单独开口,故发病机会相对较少。

2.气管切开术的手术并发症有哪些以及如何处理?
答案:①皮下气肿,一般在24小时内停止发展,可在1周左右自行吸收。
严重者应立即拆除切口缝线,以利气体逸出。

②纵隔气肿,应予胸骨上方,沿气管前下区向下分离,将纵隔气体放出。

③气胸,少量可让其自行吸收,大量行穿刺负压引流。

④出血,术后少量出血,可在套管周围填入碘仿纱条,压迫止血。
若出血
多,立即打开伤口,结扎出血点。
遇有大出血时,应立即换上带气囊的
套管或麻醉导管,气囊充气,以保持呼吸道通畅,同时采取积极的抢救
措施。
