The Multidisciplinary Management of IBC
• Induction chemotherapy (IC) followed by mastectomy and chest wall irradiation represents the standard of care for IBC • Pathological response to IC represents the most important prognostic factor • Anthracyclines and taxane-based regimens are traditionally used as IC in IBC

It is important to realize that the treatment of inflammatory breast cancer must be multidisciplinary. The disease presents diffuse and is usually inoperable at the time of presentation by definition. Therefore, induction chemotherapy is necessary in order to reduce the amount of disease that will be possibly treated with surgery followed by chest wall irradiation. Several types of chemotherapies have been tested over the years and anthracycline and taxane, most of the time, represent the basis for this regimen. And the pathological response to induction chemotherapy, in particular, the complete pathological response, or the disappearance of the tumor in the breast and the lymph node, represents an important prognostic factor for this patient. Unfortunately, only 10 to 15percent of these patients achieve that pathological response.

Incidence of Recurrence
In a retrospective analysis, it has been shown, for example, that patients treated with the proper multidisciplinary treatment, and including the chemotherapy regimen that we had just mentioned, show an increased incidence or recurrence particularly in the first two years, suggesting that macrometastatic disease is present at the time of local treatment and a completion of systemic therapy. This makes this tumor obviously more aggressive and with associated worse prognosis compared to the locally advanced non-inflammatory types.
