篇一：办理原产地证明

办理原产地证明提交原产地申请

申请单位出口每批货物时，应于货物报关出运前三天向签证机构申请办理原产地证，并严格按照签证机构的要求，真实、完整、正确地填写下列材料：

《一般原产地证明书/加工装配证明书申请书》填制申请书时，HS编码取前六位，FOB总值和拟出运日期要准确无误，要加盖申请单位印章，并由申领人签名;

《中华人民共和国出口货物原产地证明书》一式四份;

出口货物的商业发票一份;

签证机构认为必要的其他证明文件。

审核、签发原产地证

1、审核

签证机构依据中华人民共和国原产地规则、法规和有关规定，对申请单位提交的前述文件进行严格审核，符合下列条件方接受申请：

资料必须齐全、有效，单证填写完整、正确，文字清晰，印章(正、副本全部加盖)和签字无错漏;

原产地证内容须与申请书、发票一致 ;

对含进口成分的货物要提供明细单，如明细单难以查清的，申请人应提供补充说明，并接受签证机构的核查 ;

经审核，含有进口成分的货物未达到原产地标准的，应申请《加工装配证明书》或《转口证明书》。

2、签发

签证机构对申请人提交的文件审核无误后，即在原产地证第十二栏进行签证，加盖由贸促会统一刻制的"中国国际贸易促进委员会单据证明专用章"，授权签证员签字;

签证机构只签发原产地证正本一份，副本三份，其中一正二副交申请企业。另一副本、申请书、商业发票等有关文件，由签证机构存档。

3\原产地证的更改、补充及重新签发

申请单位要求更改或补充已签发原产地证的内容，必须申明更改理由并提供依据，经签证机构审查符合要求后，重新办理申请手续，收回原发原产地证，换发新证;

如果已签发的原产地证遗失或毁损，从签发之日起半年内，申请单位必须向签证机构说明理由并提供确实的依据，经签证机构审查同意后重新办理申请手续。签证机构在新签证书第五栏内加注英文"THIS CERTIFICATE IS IN REPLACEMENT OF CERTIFICATE OF ORIGINE NO. DATED WHICH IS CANCELLED."证书第十一栏和第十二栏的日期应为重发证书的实际申请日期和签发日期。未注册的单位首先根据《注册程序》申请注册登记，已在贸促会注册登记的申请单位，应由指定的原产地证申领员持贸促会颁发的《原产地证明书

申领员证》向所在地的贸促会申领由贸促会统一印制的《一般原产地证明书申请书》和《中华人民共和国出口货物原产地证明书》，并准确、真实填写其中各栏。

If more than one producer’s good is included in the certificate, list the additional producers, including name, address (including country). If the exporter or the producer wishes the information to be confidential, it is acceptable to state “Available to the competent governmental authority upon request”. If the producer and the exporter are the same, please complete field with “SAME”. If the producer is unknown, it is acceptable to state "UNKNOWN".
