无创脑水肿动态监测仪在乙型脑炎患儿的监测分析

摘要:目的探讨无创脑水肿监测仪在乙型脑炎患儿的监护应用及结果分析。方法30例乙脑患儿作为观察组,并根据入院时临床表现为初期和极期分为观察组1和观察组2,对照组为排除了颅内病变和中枢神经疾病的儿童30名。采用BORN-BE无创脑水肿监测仪分别监测乙脑患儿病程为初期(T1)、极期(T2)、恢复期(T3)及康复(T4)四个时间点的脑干扰系数,并与颅内脑水肿体积和神经功能缺损评分进行相关性分析。结果入院时观察组脑干扰系数高于对照组(p<0.05),观察组1脑干扰系数低于观察组2(p<0.05);观察组1从T1到T2脑干扰系数上升达到最大值,从T2到T4逐渐下降;未治愈乙脑患儿T2到T3间脑干扰系数没下降趋势;在T1和T2时间点脑干扰系数与脑水肿体积和神经功能缺损评分呈正相关(相关系数r分别为0.73和0.54);结论BORN-BE无创动态监护仪可客观提示乙脑患儿病程的进展和预后,在乙型脑炎发病机制研究和乙脑患儿临床监护上具有推广价值。关键词:乙型脑炎; 无创脑水肿监护仪;扰动系数;脑水肿;神经功能缺损评分Monitoring Results of children with Japanese encephalitis by non-invasive and

Yongchuan hospital, Chongqing medical university, chongqing, 402160, China Abstract: Objective to discuss the guardianship application values of noninvasive cerebral edema monitor for the children with Japanese encephalitis and analyze the monitoring results. Methods thirty children with Japanese encephalitis were selected as observation group and divided into the observation group 1 and the observation group 2 according to their clinical manifestations, thirty children without intracranial lesions and central nervous disease were selected as control group. Brain agitating coefficients of the children with Japanese encephalitis were measured at the four timings of initial stage(T1),critical phase(T2), convalescence(T3) and rehabilitation(T4) by BORN-BE noninvasive brain edema monitor, and the relevent analysis was made between the agitating coeffiecient and intracranial brain edema volume and neural functional deficit score. Results the interference coefficient of observation group was higher than that of control group (p < 0.05) and the interference coefficient of observation group 1 was lower than that of observation group 2 (p < 0.05); The interference coefficient of observation group 1 was rising to maximum from T1 to T2 during the treatment proceed and declining from T2 to T4, while the interference coefficient of no healing children with Japanese encephalitis was not declining from T2 to T3; the interference coefficient was positive related to brain edema volume and neural functional deficit score at T1 and T2 wit h correlation coefficients of 0.73 and 0.54 respectively. Conclusion the BORN-BE noninvasive dynamic monitor can be used to suggest the disease progress and prognosis of children with Japanese encephalitis objectively, it may has a promotion values for pathogenesis research and clinical monitor of Japanese encephalitis.
