淋病奈瑟菌(Neisseria gonorrhoeae)俗称淋球菌,为革兰阴性双球菌,是人类常见性传播疾病淋病的病原体。由淋病奈瑟菌感染引起的以人为唯一天然宿主的淋病是我国目前流行的发病率最高的性传染病之

一,也是我国《中华人民共和国传染病防治法》[1]规定的重点防治的乙类传染病,主要引起男性尿道炎、附睾炎、前列腺炎,女性尿道炎、阴道炎、子宫炎,以及新生儿的淋球菌结膜炎[2]。近年来,淋病发病率不断上升,其病原体淋病奈瑟菌的耐药性也日益增强。本文对2008年1月~2016年12月在我院分离到的702株

淋病奈瑟菌的检测及耐药结果进行统计与分析。

[摘要]目的检测淋病奈瑟菌临床分离株的耐药性及了解其对常用抗生素的耐药现状,对该地区淋病的临床用药治疗和控制有重要意义。方法收集2008~2016年浙江省人民医院分离的淋病奈瑟菌702株,菌落革兰染色镜检,并采用法国梅里埃VITEK 2-compact 全自动细菌检测分析系统及其配套的API-NH 细菌鉴定卡进行鉴定。采用VCAT3培养基35℃含5.0%CO 2条件下的K-B 法进行药敏检测。采用SPSS13.0统计软件进行数据分析。结果702株淋病奈瑟菌对环丙沙星和四环素的耐药性分别为91.7%和86.2%;其次是青霉素G 和氨苄西林,耐药率分别为82.7%和25.3%,对头孢噻肟和头孢曲松暂未出现耐药。结论环丙沙星、四环素、青霉素G 和氨苄西林等药物因该地区的淋病奈瑟菌的耐药性太高已不宜作为本地区治疗淋病的一线药物使用,头孢噻肟和头孢曲松等可作为首选药物考虑,同时应进一步加强对淋病奈瑟菌耐药性的监测及根据相关药敏试验合理选用抗菌药物。[关键词]淋病奈瑟菌;耐药性;抗生素;菌落革兰染色镜检;药敏试验[中图分类号]R446.5;R759.2

[Abstract]Objective To investigate the drug resistance of Neisseria gonorrhoeae(NG)clinical isolates and the drug re⁃sistance to common antibiotics,which is significant for the clinical medication treatment and control of the local gon⁃orrhea.Methods 702strains of NG isolated in Zhejiang Provincial People’s Hospital from 2008to 2016were collect⁃

ed.Bacterial colonies were tested using gram staining microscopy and identified using VITEK 2-compact automated ID/AST instrument of Biomerieux,France and matching API -NH strip.Drug sensitivity test was taken using K -B method under the temperature of 35℃containing 5%CO 2in the VCAT3culture media.Statistical analysis was taken in SPSS13.0.Results The drug resistance of these 702NG strains to ciprofloxacin and tetracycline was 91.7%and 86.2%respectively.Then was penicillin G and ampicillin,to which the drug resistance was 82.7%and 25.3%.There was no drug resistance showed to cefotaxime and ceftriaxone.Conclusion Ciprofloxacin,tetracycline,penicillin G and ampi⁃cillin are not proper to be used as the first-line drugs of local gonorrhea due to the high drug resistance of local NG.

Cefotaxime and ceftriaxone could be considered as the preferred drugs.In the meantime,the surveillance of NG drug resistance should be further enhanced and the antibiotic drugs should be selected based on the related drug sensitivity test.[Key words]Neisseria gonorrhoeae;Drug resistance;Antibiotics;Bacterial colony gram staining microscopy;Drug sen⁃
