腔隙性脑梗死的发病率及变化趋势王蔚华方向华

腔隙性脑梗死(LAC)是脑梗死的常见亚型之一,由于临

床表现较轻,近期预后较好,因此不太为临床所关注。近年

来随着研究的不断深入,发现其远期复发率和死亡率与非

LAC差别不大[1],因此,充分了解LAC的发病情况、占脑梗

死的比例及其变化趋势,对于脑梗死二、三级预防以及准确

评估脑梗死的疾病负担都有重要意义。

1965年,Fisher的病理研究确立了现代LAC的概念,他

将LAC定义为“小的深部脑梗死”。病变血管一般为脑内主

要动脉的深穿支,这些小动脉闭塞形成小腔隙软化灶,直径

2~15 mm,最大不超过20 mm[2]。
