清洁灌肠后保留灌肠多层螺旋CT扫描在直肠占位中的应用【摘要】目的:探讨清洁灌肠后保留灌肠多层螺旋ct扫描方法在直肠占位检查中的诊断价值。

方法:对51例疑有直肠占位的患者行清洁灌肠后保留灌肠ct扫描,观察直肠肠腔及肠周结构显示效果。

结果:51例患者病变显示阳性率100%,ct征象有:肠壁增厚,腔内肿块,肠腔狭窄,以及肠周邻近结构侵犯情况。

结论:清洁灌肠后保留灌肠多层螺旋ct检查对直肠占位的诊断有很大价值,良好的肠道准备及扫描方法是多层螺旋ct对直肠恶性病变诊断的关键。

方法选取2021年7月—2023年5月新沂市人民医院收治的89例肺结节患者为研究对象,对所有研究对象予以胸部螺旋CT平扫和靶重建技术,收集并分析不同检测方法的准确度、灵敏度及特异度。

结果 CT靶重建技术准确度为97.75%(87/89)、特异度为97.14%(34/35)、灵敏度为98.15%(53/54),高于胸部螺旋CT平扫,差异有统计学意义(χ2=19.264、12.857、7.053,P<0.05);CT靶重建技术影像特征符合率高于胸部螺旋CT 平扫,差异有统计学意义(P<0.05)。

结论在针对肺结节良恶性进行诊断时,选择CT靶重建技术进行检查能够进一步提高检测的准确度,能够更准确地检测出病灶影像特征,检测图像清晰度更高。

Chest spiral CT plain scan and target reconstruction were performed on all subjects, the accuracy, sensitivity and specificity data of different detection were collected and analyzed. Results The accuracy, specificity and sensitivity of CT target reconstruction were higher than those of CT plain scan, and the difference was statistically significant (χ2=19.264, 12.857, 7.053, P<0.05). The accuracy of CT target reconstruction was 97.75% (87/89), the specificity was 97.14% (34/35) and the sensitivity was 98.15% (53/54). The coincidence rate of CT target reconstruction was higher than that of chest spiral CT plain scan, and the difference was statistically significant (P<0.05). 多层螺旋CT血管成像动态图像后处理对脑血管畸形病变的诊断价值研讨王丽,周田,刘斯咪[摘要]目的研究多层螺旋CT血管成像动态图像后处理运用于脑血管畸形病变患者中的效果。

方法随机选取2020年10月—2023年10月聊城市人民医院接诊的70例疑似脑血管畸形病变患者为研究对象,将数字减影血管造影结果作为诊断金标准,并且提供多层螺旋CT血管成像动态图像后处理,应用CT多平面重组、三维容积漫游及联合诊断分析诊断效能。

结果数字减影血管造影阳性检出率为48.57%,CT多平面重组诊断阳性检出率为42.86%,三维容积漫游诊断阳性检出率为44.29%,联合检查阳性检出率为51.43%。

CT多平面重组诊断、三维容积漫游诊断的灵敏度、特异度、准确度相比,差异无统计学意义(P均>0.05)。

联合诊断的灵敏度、准确度显著高于CT多平面重组诊断,差异有统计学意义(P均<0.05)。

三维容积漫游诊断、联合诊断的灵敏度、特异度、准确度比较,差异无统计学意义(P均>0.05)。

结论多层螺旋CT血管成像动态图像后处理中,CT多平面重组联合三维容积漫游诊断运用于脑血管畸形病变患者中具有较高的诊断效能,诊断价值高。

The results of digital subtraction angiography were used as the diagnostic gold standard, and dynamic image processing of multi-slice spiral CT angiography was provided, application of CT multiplanar reconstruction, three-dimensional volume roaming and combined diagnosis to analyze diagno⁃sistic performance. Results The positive detection rate of digital subtraction angiography was 48.57%, the posi⁃tive detection rate of multi-plane CT recombination diagnosis was 42.86%, the positive detection rate of three-dimensional volume wandering diagnosis was 44.29%, and the positive detection rate of combined examination was 51.43%. There was no significant difference in sensitivity, specificity and accuracy between CT multi-plane recombination diagnosis and three-dimensional volume roaming diagnosis (all P>0.05). The sensitivity and accuracy of combined diagnosis were significantly higher than that of CT multi-plane recombination diag⁃nosis, and the differences were statistically significant (all P<0.05). 腰椎狭部裂的螺旋CT诊断价值【摘要】目的探讨ct在腰椎椎弓峡部不连诊断中的临床价值及提高检出率,提高对该病的认识和诊断水平,方法搜集腰椎螺旋ct容积扫描,经后处理重建检出的腰椎峡部裂55例,分析其mpr、vr成像的ct表现,并与常规椎弓ct轴位像进行对照,评价对峡部裂及其继发病变的显示情况。

结果 mpr冠状面、经椎弓矢状面重建结合vr成像可清晰显示55例108处峡部裂; mpr结合vr图像重建对峡部裂特征以及裂隙骨赘、椎间孔狭窄、椎体滑脱、椎间盘后突的相关继发病变征象显示均有明显优势。

多排螺旋CT在骨关节创伤中的诊断价值【摘要】目的:探讨16层螺旋ct三维重建技术在骨关节创伤性疾病中的诊断价值。

方法:骨关节创伤92例,伤患处均行16层螺旋ct横断面扫描,在工作站行三维重建技术后处理,即表面遮盖显示(ssd)、多平面重建(mpr)及容积重建(vr)技术,与x线平片比较,评价3种重建方法对骨折或脱位的显示效果。

结果 16层螺旋横断面检出91例,mpr重建对所有骨折的部位及形态全部明确诊断,获ssd重建技术明确诊断89例,获vr重建技术明确诊断90例。

经16层螺旋ct横断面扫描可发现细小骨折,但立体信息不足;三维重建技术可多角度、全方位观察骨折部位的骨结构空间关系,ssd能立体、逼真地显示骨折及脱位情况,mpr能兼顾软组织的改变,在骨折线走行和移位程度具有优势,在显示细小的骨折线上更具优势。

结论:各种三维重建技术各有优势,与16层螺旋ct联合使用可提高对骨关节损伤诊断的准确性,为临床诊治提供更多准确、直观的信息。

方法随机选取2020年1月—2023年10月济宁市第三人民医院收治的80例结肠癌患者为研究对象,均予多层螺旋CT检查,以手术病理结果为依据,评估CT对结肠癌术前TNM分期的诊断准确率及与病理结果的一致性。

结果在结肠癌术前分期诊断中,多层螺旋CT对T1、T2、T3、T4期诊断符合率80.00%、91.67%、100.00%、93.33%,T分期准确率93.75%;对N0、N1、N2期诊断符合率88.00%、85.29%、95.24%,N分期准确率88.75%;对M分期诊断准确率91.25%;对TNM分期诊断结果与手术病理结果一致性较好(kappa= 0.91、0.83、0.81)。

结论结肠癌患者术前予多层螺旋CT检查TNM分期诊断准确率高,且与手术病理结果一致性较好。

Methods A total of 80 patients with colon cancer admitted to the Third People's Hospi⁃tal of Jining from January 2020 to October 2023 were randomly selected as the research objects. All patients were examined by multilayer spiral CT. Based on the surgical pathological results, the diagnostic accuracy of CT for preoperative TNM staging of colon cancer and the consistency with pathological results were evaluated.Results In the diagnosis of preoperative staging of colorectal cancer, multilayer spiral CT had a diagnostic con⁃cordance rate of 80.00%, 91.67%, 100.00%, and 93.33% for T1, T2, T3, and T4 stages, and an accuracy rate of 93.75% for T staging. The diagnostic compliance rate for N0, N1, and N2 stages was 88.00%, 85.29%, and 95.24%, and the N staging accuracy rate was 88.75%. The diagnostic accuracy rate for M staging was 91.25%; the diagnostic results for TNM staging were in good agreement with the surgical pathology results (kappa=0.91, 0.83, 0.81). 186泌尿系梗阻性疾病在临床发病率很高,泌尿系梗阻性疾病发作时,常因病因诉说不清,或临床症状不典型而容易延误诊断,随着CT 技术的进步,MSCT 大范围扫描,多层螺旋CT 尿路造影(MSCTU)的应用,可多方位观察双肾、输尿管及膀胱等病变的形态,能在定位、病程发展和并发症方面给泌尿系梗阻性疾病的诊断提供更多的影像学依据,为临床选择恰当的治疗方案提供影像学依据。

1 资料与方法1.1 一般资料 选取本院2005年2月—2011年6月行MSCT 检查的泌尿系梗阻性疾病患者62例为观察组,男42 例,女20例,男女之比为2:1,<30岁23例,30~50岁25例,>50岁14例,平均年龄38.4岁。

1.2 仪器 西门子16排螺旋CT 机,电压为120kV,电流为150mA。

1.3 MSCT 扫描方法 自第12胸椎至耻骨联合水平,层厚:5mm,1次闭气完成扫描。

根据病人状态可选择以下几种方法:①非增强的螺旋CT 检查法:在1次摒气下,连续扫描多层,不需要对比剂;②静脉注入对比剂法:平扫和三期动态增强扫描。

增强方式:从上肢静脉经高压注射器注入欧乃派克80~100mL,注射速率2.5m/sL,分别于开始后30s(皮质期)、90s(实质期)、180s(排泌期)进行三期增强扫描,必要时增加延时15~30min 扫描。

1.4 图像后处理技术 将各期原始数据1mm 重建,分别采用最大密度投影(MIP )、多平面重建(MPR )、曲面重建(CPR )和容积再现(VR )等方法进行图像处理。

2 结 果62例病例中38例诊为肾盂或输尿管结石,平扫时即可发现,CT 值:150~1800Hu,CTU 表现为腔内高密度影,梗阻上方输尿管及肾盂不同程度扩张。

9例为输尿管狭窄,CTU 表现为输尿管狭窄段呈突然狭窄或渐进性,但无明显管壁增厚,伴有狭窄段上方输尿管扩张积水;6例为肾盂或输尿管肿瘤,MSCTU 检查表现为肾盂内乳头状软组织影,输尿管壁局限性增厚、狭窄,伴有输尿管扩张积水,增强扫描实质期病变中度强化;5例为膀胱肿瘤,病变位于膀胱三角区附近,增强扫描实质期肿块略有强化,延迟期膀胱内可见肿块样充盈缺损;2例肾盂输尿管畸形;2例输尿管外压性梗阻,均为转移瘤所致鸟嘴样狭窄,多层螺旋CT三维尿路造影(MSCTU )诊断泌尿系梗阻性疾病临床价值吴涛(辽宁中医药大学附属二院医学影像科,辽宁 沈阳 110034)摘 要:目的:探讨螺旋CT 对临床表现泌尿系梗阻性疾病的诊断价值。

在影像学中,肋骨骨折的X 射线片和CT 片的成像特点不同,由于X 射线检查的灵敏度和肋骨解剖学的结构特殊性,对于有错位的骨折可诊断明确,比较轻微和重叠区的骨折难以及时发现,而且由于患者体位、投照位置和角度、以及条件限制,传统X 射线片难以充分地展示骨折的部位和数目,易发生漏诊;CT 检查优于X 射线检查之处在于其密度分辨率高,图像清晰,解剖关系明确,能提供没有组织重叠的横断面图像,且三维重建可进行冠状面、矢状面以及三维立体图像的重建,可以更逼真地显示病变的部位图像,弥补了X 射线片和常规CT 检查的不足,提高了骨折诊断的准确性[1]。

为了研究探讨隐匿型肋骨骨折在64排螺旋CT 三维重建的检查与应用价值,为临床诊疗及法医鉴定提供准确的诊断依据。

对2015年1月~2016年12月来本院诊治的96例胸部外伤患者相关资料进行分析,报道如下。

3.0TMRI-MRCP、多层螺旋CT、B超对胆道系统结石诊断价值分析【摘要】目的:探讨3.0TMRI-MRCP、多层螺旋CT、B超三种检查对胆道系统结石的诊断价值。

方法:回顾性分析2020年12月~2021年10月在本院经ERCP或手术证实的93例胆道系统结石病例。

对术前MRCP、CT及B超检查的影像学资料采用25.0 SPSS行卡方检验等分析。

结果:1.胆道系统结石总检查率:MRCP检出率>CT检出率> B超检出率,三者差异具有统计学意义(χ2=9.803,P=0.007)。

2.MRCP在胆总管结石、肝内胆管结石或单发性结石的检出率明显优于B超。

3.MRCP检查对直径≤10mm的小结石检出率明显优于B超检查,差异具有统计学意义(χ2=9.345,P=0.002)。

结论:在胆道系统结石影像检查诊断中,特别在胆总管结石、肝内胆管结石、单发性结石及直径≤10mm的小结石中,3.0TMRI-MRCP评估准确性及临床应用价值方面有更明显的优势。

The total detection rate of biliary tract stones: MRCP detection rate > CT detection rate > B-ultrasound detection rate, and the difference was statistically significant (χ2=9.803,P=0.007).2.The detection rate of MRCP in common bile duct stones, intrahepatic bile duct stones or single stones was significantly better than that of B-ultrasound.3. CHINESE COMMUNITY DOCTORS中国社区医师2021年第37卷第15期现如今生产力以及经济发展迅速,建筑业、交通业也快速发展,发生交通事故伤、摔伤等会对人们的身体健康及生活质量造成影响。

胸部外伤具有较高发病率,在外科疾病中占比较高[1]。

有关研究证实,准确诊断可促进患者治疗和康复,提升治疗效果,将预后效果予以改善[2]。

对2018年2月-2020年2月收治的胸部外伤患者52例进行64排螺旋CT三维重建技术诊断的价值作用研讨。

资料与方法2018年2月-2020年2月收治胸部外伤患者52例,男24例,女28例;年龄19~61岁,平均(40.32±2.15)岁;依照临床调查结果显示,重物砸伤损伤13例,高处坠落伤损伤10例,交通事故伤损伤29例。

所有患者及家属均签署知情同意书,本研究经伦理委员会批准。

方法:所有患者均实施X 线、64排螺旋CT 三维重建技术诊断。

①X 线片:使用法国THALES 公司生产的DR 摄片机进行诊断,型号为Pixium 3542EZ,对患者实施正斜位、胸部正位X 线检查。

②CT:使用飞利浦公司生产的16排螺旋CT,型号为Mx16-Slice,对患者实施检查,调整患者体位,保持仰卧位,自肺尖上3cm 部位实施扫描,扫描至肋骨下缘,对全部肋骨实施扫描,管电流为500mA、管电压为120kV,设置扫描层厚为12mm、螺距为0.25、扫描周期为0.5s、重建层厚为3mm,扫描后净化数据传输至工作站对工作进行重建。

观察指标:比较两种诊断方式的诊断结果。

统计学处理:数据应用SPSS 20.0软件处理;计数资料以[n (%)]表示,采用χ2检验;P <0.05为差异有统计学意义。

结果两种诊断方式的诊断结果比较:CT 诊断肺挫裂伤以及肋骨骨折检出率均高于X 线诊断,差异有统计学意义(P <0.05);两种诊断方法的肩胛骨骨折、锁骨骨折检出率比较,差异无统计学意义(P >0.05)。

Methods: A total of twenty-eight patients with thoracic and abdominal aortic disease were selected to undergo 3DCE-MRA examination with conventional T 1-weighted image (T 1WI) and T 2-weighted image(T 2WI) of plain scanning axis position. Low-dose contrast agent was adopted to undergo single layer dynamic scanning of axial position so as to obtain the delay time of scan. And then, the plain scan collected coronary images of target blood vessels as the mask of post subtraction. The obtained delay time in the last step was used to undergo 3DCE-MRA. After contrast agent was injected, the coronary images of target blood vessels and the subtraction of mask pre enhancement were respectively collected at arterial and venous phases. At the last, the axial fast spin echo (FSE) T 1WI post enhancement was collected, and the signal performances of vascular lesions of thoracic and abdominal aorta were observed. Results: In the 28 patients, 5 cases were aorto-arteritis (17.90%), and 1 case was lilac artery aneurysm (3.6%), and 3 cases were atherosclerosis (10.7%), and 6 cases were renal artery stenosis (21.4%) and 13 cases were aortic aneurysm (46.4%). The display effect of 3DCE-MRA technique was favorable for the aortic vessels and branches of chest and abdomen of 28 patients. 方法 选取河北北方学院附属第一医院2019年1月至2020年1月期间120例乳腺肿块性病变患者,经多层螺旋CT灌注扫描,其中,乳腺癌63例,良性肿块57例。

测量分析血流量(BF)、血容量(BV)、平均通过时间(MTT)、血管表面通透性(PS)等动态图像数据,依据乳腺癌与良性肿块划分不同组别,实施统计学分析。

结果 乳腺癌组患者的BF、BV、PS均高于良性肿块组,两组差异有统计学意义(P 0.05);两组患者的MTT差异无统计学意义(P 0.05);乳腺癌患者的毛细血管数量增多,管径较正常宽,使微血管容量增加,血流加快。

良性肿块患者的微血管密度与乳腺癌实质相似或轻度增多,微血管管径与正常乳腺微血管管径相同,灌注的微血管与非灌注的微血管比例增加不明显。

结论 在乳腺肿块诊断中实施多层螺旋CT灌注成像扫描,准确率高,利于鉴别、诊断乳腺癌。

材料与方法:回顾性分析89例诊断为原发性三叉神经痛(单侧疼痛)患者,均接受Siemens 3.0T磁共振三叉神经成像的检查(包括T1-VIBE序列和T2-SPC序列),由两位副高级以上影像医师共同分析病例中三叉神经微血管压迫的影像学征象,并从中归纳三叉神经与邻近血管的关系,评价其显示三叉神经脑池段(REZ)与周围血管关系的能力和优势。

结果:89例患者中,症状侧血管神经Ⅰ型32例,Ⅱ型47例,Ⅲ型10例;而无症状侧Ⅰ型74例,Ⅱ型14例,Ⅲ型1例;双侧压迫程度差异显著,存在统计学意义(P<0.001)。

而在有血管神经接触、压迫情况下,症状侧57个病例中,小脑上动脉为主要的责任血管(63%)。

结论:T2-SPC序列能清晰显示三叉神经与周围结构的关系,T1-VIBE序列是三叉神经MR成像常用的补充序列,T2-SPC与T1-VIBE序列相结合能提供准确的诊断信息,对原发性三叉神经痛的诊断具有重要价值。

胸腹部CT联合增强扫描方式分析赵海波【摘要】目的回顾性探讨胸腹部肿瘤CT联合增强扫描时间的选择及扫描方式的优化,更好地显示病变部位及周围组织情况.方法选取2010年1月至2011年7月间行胸腹部CT增强检查的患者120例,分为A、B、C三组:A组,扫描期相为肝脏动脉期、肺部、肝脏静脉期,扫描方向为头足向;B组,扫描期相为肝脏动脉期、肺部+肝脏静脉期,扫描方向为头足向;C组,扫描期相为肝脏动脉期+肺部、肝脏静脉期,扫描方向为足头向.三组患者图像以双盲法评价分级.结果 C组的图像质量明显优于A组和B组,三组间比较差异有统计学意义(P<0.05),A组与B组比较差异无统计学意义(P>0.05),A组与C组比较、B组与C组比较差异均有统计学意义(P<0.05).结论胸腹部肿瘤患者进行胸腹部CT联合增强扫描时,在常规扫描参数相同的前提下,肝脏动脉期+肺部、肝脏静脉期,足头向的扫描方向为最佳扫描方式,以利于肿瘤的诊断及临床分期.%Objective To retrospectively investigate the optimization of scan mode and the selection of scan time in the combined thoracic and abdominal tumor enhanced CT. Methods One hundred and twenty patients tested by the chest and abdomen enhanced CT examination were selected in Liuzhou Worker' s Hospital from January 2010 to July 2011. They were divided into A,B and C groups, A:liver arterial phase,lung,liver venous phase, scanning direction: head to foot; B: liver arterial phase, lung and liver venous phase, scanning direction:head to foot;C :liver arterial phase and lung,liver venous phase,scan direction:foot to head. The CT images were evaluated by double-blind trials. Results The image quality of C group was better than A and B group. The differencesamong the three groups were statistically significant ( P < 0. 05 ). The differences between A group and B group were not statistically significant (P > 0. 05 ). The differences between A group and C group, B group and C group were statistically significant ( P < 0. 05 ) . 结果:肾癌3种亚型CT平扫值在统计学上无显著性差异,非透明细胞癌(乳头状肾癌和嫌色细胞癌)在多期增强的皮质期、实质期及延迟期的CT值要低于透明细胞癌(各期P值均为0.000),非透明细胞癌在皮质期、实质期及延迟期的肿瘤/肾皮质CT值的比值与透明细胞癌间有统计学差异(各期P值均为0.000),乳头状肾癌与嫌色细胞癌在增强扫描各期的CT绝对值间比较无统计学差异(P值分别为0.376、0.315、0.382),但在皮质期、实质期及延迟期肿瘤/肾皮质CT值之比值上乳头状肾癌与嫌色细胞癌之间有统计学差异(各期P值分别为0.046、0.031、0.048)。

结果 CT检查对脉粥样硬化斑块及颈动脉狭窄的诊断符合率均高于MR检查,但对比无统计学意义(P>0.05)。

结论 CT与MR均可明确显示颈动脉狭窄和粥样硬化斑块详情,但就诊断符合率来说CT检查更高,有助于脑血管疾病的“早发现、早诊断、早治疗”。

Methods 3D-Vibe technology (by Siemens Magnetom Verio 3T MRI System) was applied in 23 patients with trigeminal neuralgia. Results All the cases displayed clear images showing the trigeminal nerve and vascular compression, which reached the clinical diagnostic criteria. Twelve patients were confirmed through follow-up. 螺旋CT肺灌注显像对肺气肿分析与评价摘要】目的:探讨螺旋CT肺灌注显像对肺气肿的诊断价值。

方法:选择我院治疗的慢性阻塞性肺气肿患者27例作为观察组,选择同期27例健康体检者作为对照组,均采用螺旋CT肺灌注显像,观察两组血流量、血容量、平均通过时间和肺表面通透性情况。

结果:观察组患者在血流量、血容量和表面通透性情况均低于健康体检者,组间对比差异有统计学意义(P<0.05)。

结论:CT灌注显像属于无创诊断与评价的方法,能够为早期诊断慢阻肺提供相应的依据,但在技术层面需要进一步探讨。

【关键词】螺旋CT 肺灌注显像肺气肿【中图分类号】R445 【文献标识码】A 【文章编号】1672-5085(2013)46-0013-02The analysis and evaluation of Spiral CT perfusion imaging in pulmonary emphysema【Abstract】 Objective To explore the diagnostic value of spiral CT pulmonary perfusion imaging of pulmonary emphysema.Methods Choose the hospital treatmentof 27 patients with chronic obstructive pulmonary emphysema as observation group, select the 27 cases of healthy physical examination for the same period as the control group, adopt spiral CT pulmonary perfusion imaging, observe two groups of blood flow, blood volume, average and pulmonary permeability surface through time.Result Observation group of patients in blood flow, blood volume and surface permeability were lower than healthy check-up, comparing differences between groups was statistically significant (P < 0.05).Conclusion CT perfusion imaging is a noninvasive diagnostic and evaluation methods, to provide corresponding basis for early diagnosisof copd, but in the technical level to be further discussed.【Key words】 Spiral CT perfusion imaging pulmonary emphysema慢性阻塞性肺气肿属于临床上呼吸内科的常见疾病,是以不完全可逆性的气流受限为主要特征的一类疾病,由于患者存在慢性气道炎症改变会引发气道狭窄形成阻塞性细支气管炎,还会造成肺实质的损伤,导致了肺脏弹性回缩能力下降[1]。
