Problem : Describe problem simply Why 1 Give answer for the question ‘why the problem above happened?’. It identifies the cause of the problem above. Why 2 Give again answer the question to the same question ‘why the answers in left happened?’ Why 3 Give again answer the question to the same question ‘why the answers in left happened?’ Contribution% Sum should be 100% Therefore (Root Cause) Repeat until you cannot answer the ‘why’ anymore, which will be the root cause of the problem in process level More than one column can be used if there’re multicauses per level Repeat until you cannot answer the ‘why’ anymore, which will be the root cause of the problem in detection level %

Simulation to verify root cause is optional depending on availability. But, it is highly recommended to go through the simulation to validate if the problem is reproducible under the estimated/identified root cause condition.

• Existing lots (WIP) at Finish Goods that are potential affected should be recalled/pulled out, checked and verified. Lots affected by the problem (based on Amkor feedback) should be identified for proper disposition. • Existing lots (WIP) at the station and after the station where the problem might have happened should be identified, recalled/pulled out, checked and verified for possibility of having the defects as feedback by Amkor (this is a Product OCAP implementation). • If necessary, the checkpoints at in-process monitors and/or gates should be tightened adequately to capture the defect/problem in preventing defectives from reaching the customers. Note: - Results of the lot checking/verification must be included in this portion. - Supplier containment actions must be in effect until permanent actions are put in place. - Supplier must support and pay the sorting or any activity done on their discrepant materials since they are liable and responsible for that containment action performed by Amkor due to supportability issue.
