头皮屑患者中马拉色菌感染的研究

目的:探讨头皮屑中马拉色菌的带菌情况、菌种构成及马拉色菌感染与头皮屑严重性的关系。方法:采用Leeming和Notman培养基、Dixon培养基和含菜子油培养基分别同时培养232份的头皮屑,观察培养阳性率与头皮屑严重程度的关系,根据生理生化学及形态学特点鉴定菌种,并比较在这3种培养基中菌落初长时间及对菌种有无选择性。结果:232人中189人的头皮屑中分离到马拉色菌,阳性率为81.47%,培养阳性率与头皮屑的严重程度有正相关关系。3种培养基共分离到481株马拉色菌,从中鉴定出合轴马拉色菌、球形马拉色菌、限制马拉色菌、糠秕马拉色菌和钝形马拉色菌共5个种。3种培养基对菌种无选择性,但在含菜子油的培养基中菌落初长时间最短。分离到单一菌种的头皮屑标本有155份(66.81%),分离到2种菌种的有24人(12.70%),分离到3种菌种的有9人(14.94%)。结论:头皮屑中马拉色菌菌种检出率较高,优势菌种主要为合轴和球形马拉色菌,部分头皮屑患者存在2个、甚至3个马拉色菌种感染,头皮屑严重程度与马拉色菌感染密切相关。

【Abstract】Objective To investigate the distribution and the constitution of the Malassezia genus in the dandruff patients.Methods Each dandruf sample collected from 232 dandruff patients was divided into three parts and inoculated in Leeming and Notman medium, Dixon medium and the medium containing rapeseed oil.The yeasts obtained from the cultures were identified based on the biochemical, morphological characteristics, and molecular genotyping.The duration from inoculation of dandruff to the primary colony development of Malassezia genus and the species constitution of Malassezia were compared among above three media.The relationship between the severity of the dandruff and the rate of isolation of the Malassezia species was also studied.Results Among 228 samples,Malassezia was isolated from 189 persons’dandruff samples(81.47%).There was a correlation between the positive rate of cultivation and the severity of dandruff. Totally 5 species were identified from 481 strains of the genus Malassezia obtained from three kinds of media mentioned above, Including M.sympodialis,M.globosa,M.restricta,M. furfur, and M. obtusa.No species differenee was noticed in the three kinds of culture media, but the colony of Malassezia grew faster in the medium containing rapeseed oil (average 5.95 days)than the other two media.Single species was isolated in three media from 155 dandruff samples(66.81%),while two species were isolated from 34 samples (14.94%),and three species were isolated from 9 samples(4.76%).Conclusion The major species of Malassezia in dandruf are M. sympodialis and M. globosa,Some patients are coinfected with 2, even 3 species of Malassezia. The severity of dandruff is positively associated with Malassezia

infection.【Key words】fungus; Malassezia;skin disease:dandruff 马拉色菌属于条件性致病菌,可以引起多种疾病,尤其是皮肤病。因此,近几年对它的致病因素有了越来越广泛的研究。大量研究表明,马拉色菌引起的皮肤疾病是有多种因素综合作用的结果,疾病的发生与该菌的密度,菌种及代谢产物均有密切关系[1,3]。

大量研究表明,马拉色菌可以引起花斑癣、糠秕孢子菌毛囊炎、脂溢性皮炎及头皮屑,特应性皮炎和银屑病等。随着研究领域的不断扩大,也有研究证实它与甲真菌病及系统感染有关[1,3,4,5,6]。

此外,还有报道马拉色菌可引起糠秕孢子菌性真菌血症,新生儿痤疮和脓疱病,甚至发现湿疹和荨麻疹中也存在马拉色菌。并且随着胃肠外营养的广泛应用,此菌可引起肺等深部系统感染的病例也有报道[1]。

有关头皮屑中马拉色菌菌种的构成和优势菌种的报道较少,结论也不尽一致。因此,我们对收集的头皮屑标本进行马拉色菌的分离和鉴定。为避免不同培养基可能出现的菌种选择性分离,将来自同一人的头皮屑标本分成3份,分别接种在国内外已报道的3种培养基上,比较对培养阳性率、菌落初长天数和菌种构成的影响,探索马拉色菌感染与头皮屑严重性的关系。

1 材料和方法

1.1 研究对象: 232份头皮屑标本采集自云南三家医院的皮肤科。年龄17~777岁,平均31.58岁。纳入标准:2周内头皮未外用过抗真菌药物或皮质类固醇制剂;2个月内未口服过抗真菌药物;不伴有特应性皮炎、银屑病以及其他头皮皮肤病。记录头皮屑严重程度(洗头后第4~5天)。

1.2 培养基:①Leeming和Notman培养基(LNM);②Dixon培养基:3.6%麦芽浸膏、0.6%蛋白胨、2%干燥牛胆汁、1%吐温40、1.2%琼脂、0.2%甘油、0.2%油酸、0.5%放线菌酮和0.05%氯霉素;③含菜子油培养基。

1.3 仪器与试剂

1.3.1仪器设备: PCR仪,台式小型高速离心机,台式高速冷冻离心机,紫外分光光度计,紫外透射仪,-80℃低温冰箱,-30℃中温冰箱,电泳仪,恒温培养箱,水浴箱,恒温摇床,显微镜,超净工作台,生物安全柜。

1.3.2PCR试剂盒,细菌DNA提取试剂盒,DNA纯化试剂盒。

1.3标本收集和培养:于2011年6~11月完成。用无菌镊子分别收集头皮屑(洗头后的第4~5天)于1.5ml样品管中,将每份头皮屑磨碎,平均分为3份,分别接种于上述3种培养基平板(直径75 mm平皿)上,32℃培养7~14 d。挑取每种培养基上生长的单菌落,经50%派克墨水一10%KOH染色,镜下确定为马拉色菌后,根据菌落的颜色、形态等特征,每个平板至少挑取3个单菌落在同种培养基平板上画线培养,再取单菌落于同种培养基斜面上增菌保存。

1.4 菌种鉴定

1.4.1 形态学和生理生化特性的鉴定:采用Guillot报道的对该菌形态学和生理生化特性的鉴定方法进行鉴定,以7个标准菌株作为阳性对照。

1.4.2 如果分离物无法以形态学和生理生化特性进行鉴定,则采用分子生物学技术技术进行种系分类鉴定。PCR所用引物如下:(1)D1/D2 LSU引物:NL1 (forward) GCATATCAATAAGCGGAGGAAAAG,Tm 65.3 ℃和NL4 (reverse) GGTCCGTGTTTCAAGACGG,Tm65.5 ℃。(2)ITS region引物:ITS1 (forward) TCCGTAGGTGAACCTGCGG,Tm68.4 ℃和ITS4 (reverse) TCCTCCGCTTATTGATATG,Tm 57.6 ℃。
